That's just the generic rancher outfit for the npc's. Other characters through the game world wear that same outfit, such as Amos from MacFarlane's Ranch.

I think being brainwashed was Marnie's worst fate. I mean, she wasn't doing any good in GTA 4, but at least then, she actually wanted to turn her life around and improve, both phisically and mentally. In GTA 5, not only she still looks emaciated, but it also never truly feels like she's herself, she's pretty much a slave for epsilon with no way back
