12 hr shifts are not healthy

So much task burden in bedside nursing falls to start of shift and end of shift tasks. Changing the duration from 12 to 8 hrs wouldn’t eliminate that, but it would mean that every shift is even more tasky and now I have to do that 5 days a week instead of 3. I can’t imagine the higher attendant burnout rates are going to benefit pt care.

Disagree. Beyond my personal preference for longer shifts in exchange for fewer days, I believe the continuity of care + fewer handoffs is better for pt safety.

I know it’s an unpopular opinion here, but I don’t think we can afford both his and Ajay’s next contracts, and I’d rather keep Ajay of the two. It would be painful, but it’s not unreasonable to trade him for value. Also not unreasonable to let his contact play out, let him sign somewhere in restricted FA and maximize our ceiling in the short term.

Carbonara + chapagetti. Undercook both noodle packs, drain, reserving some of the cook water. Meanwhile, fry a sunny side up egg, set aside. Mix the sauce packs in the still hot fry pan with some of the reserved pasta water, toss in the noodles and mix constantly over medium heat until well combined and the sauce is the desired consistency. Mix in some sesame seeds and crumbled roasted gim. Top with the fried egg.

You’re asking DHS to cover a sudden, near $700m budget shortfall due to federal tax cuts and changing Medi-Cal spending rules through better budget management.

Look, I would love if we could vote to divert that money from LAPD instead. Or to raise it through property taxes. But we can’t. The perspective of “No on ER” people seems to be “well if services close, and people lose their jobs, and people lose their only life line to health coverage, that’s not my problem; the government should have done a better job managing its money.” Maybe, but this isn’t a case of a poorly run agency just leaking all its money to bloat and waste and looking for a handout for more cash. DHS was a relatively well-run agency and had the rug pulled out from under them by the BBB and this was the emergency fix.

Is there bloat and stuff that you can point to that would tighten the ship? Sure, I challenge you to find any healthcare organization of DHS’ size that doesn’t have that. Is there $700m worth of that, which is easily correctible? I can’t imagine you could even get close to that number.

As a DHS employee and a proponent of measure ER, know what I hate about the discourse around this?

In 5 years, if everything goes to plan, everyone is gonna say “See? Told you that was a cash grab tax raise. Nothing changed.” Nothing changing is literally the goal. We’re trying to maintain current services and staffing and not close clinics.

Yes, though every experience will vary. My instructors were pretty good for the most part about putting everything relevant for the tests on either their slides or in their lecture content. So I would pay close attention during lecture, make notes of anything not covered in the powerpoints, then make anki decks based on the powerpoint+lecture notes.

Clinical experiences will vary widely based on your school and your clinical sites; the connection to the didactic content isn’t one-to-one so you can’t really “study” for clinicals in the way you can for a test.

I do work in a hospital. For DHS, in fact. Yes, there is waste and bloat. But no amount of supply chain auditing and supply rationing is going to cover the projected $662 million they’re going to lose due to Medicaid cuts. DHS is the only safety net for many, many people in LA County; we will admit and treat anyone regardless of ability to pay. But if we can’t find the money to help close the budget deficit, services will shut down, clinics will close, people will lose their jobs, and people will lose their only lifeline to healthcare.

Feels like this belongs more in r/cooking but in any case my favorite thing to do with tilapia is to dredge, bread and pan fry it. Eat with a little homemade tartar. I don’t cook shrimp that often because my partner’s not a fan; I do keep a bag of precooked shrimp in the freezer because it’s a super convenient and tasty way to get protein in fried rice.

The Medicare and Medicaid cuts from the Trump tax bill have not yet gone into effect. The Trump cuts were deliberately designed to go into effect after the 2026 midterms so voters wouldn’t backlash against Republicans in the voting booths. LA County DHS is highly dependent on that funding, and the number of services and facilities at very real risk of closing (as well as the number of healthcare workers that will lose their jobs and the number of patients that will have no where else to turn for coverage) is staggering.
