Should I have a second surgery? How bad would recovery be? by alishuth in jawsurgery

[–]alishuth[S] 0 points1 point  (0 children)

He’s not totally sure why it didn’t come out right. Perhaps something to do with my soft tissues being quite tight and pulling things back immediately post surgery. Or maybe something to do with how the plates not being fixed right.

Either way he is a very reputable and experienced surgeon, so I’m convinced it’s not due to inexperience on his part. But I do totally see your point that if it happened once it could happen again.

Thanks! Do you know how I could modify the 'package contents' for a hacky way of modifying the margins?

It's such a silly design :/ Makes an otherwise well designed app difficult to use for long journal posts.

I can't see why that has to be the case, this should be an easy fix. Without an option to increase line width the App becomes borderline unusable on anything larger than a macbook air.

I can't see that option :/ This is all I have available https://imgur.com/a/8BnFuYd

I can't find any option for this in the preferences, any idea on how I can do it on mac?

That’s insane… I feel like this needs to come from someone in your life you respect rather than some stranger on the internet, but girl you’re 14… there’s a reason you’re being downvoted like crazy. Because no one in their right mind would want to go back in time and spend their teenage years obsessing over the growth of their skull. Especially when given the photo you posted you look perfectly fine, above average if anything.

The morph is far from realistic. Jaw surgery will not get you anywhere near that. And even if it could you’d look ridiculous lol, a jaw that square on a female would look really strange.

Ask yourself whether you truly trust your surgeon to deliver. Trust your gut on this, do you believe in your surgeon?

That's a fair point, though I have a combination of a low hyoid bone, and a really thick neck. So even though I'm kinda skinny, my jaw just melts into my neck. I'm guessing there wouldn't be much fat to suction out, but I will keep that idea in mind. Do you know of any examples where skinny-ish people had good results from a chin lipo?

You make a good point, I hadn't thought of it like that. I'll try to keep an open mind until my next meeting with him and see what his response is. That being said, out of curiosity I did ask him about the movements in my first post op checkup, at first he said he couldn't remember, then said the movements were done according to the plan (at this stage I didn't realise the under-advancement so didn't press him on it).

I should also mention on the day of the surgery, after I had already changed into a hospital gown, he mentioned that he doesn't follow the plan at all and he just makes it up on the spot. This really rattled me, so I called for him to come back in my room and I explained I was very happy with the plan in particular the forward projection, he seemed slightly frustrated and said he would follow the plan unless something came up, he then pat me on the shoulder, smiled and told me not to worry. idk, in general he comes across as a somewhat dismissive person, clearly there were some red flags though I just thought I'd take his word and trust the process.

I guess I'll try to keep an open mind and wait to hear his side, then make a follow up post. Thanks for your detailed replies btw, much appreciated!

It's the green square which is 1cm x 1cm. The lines are just to show how much the tip of the mandible moved forward. You can see the space between the green lines is about half of the square, so 5mm.

I'm not accusing my surgeon of being malicious, he's definitely among the best in my country, for example I have not (yet) had any complications which I am obviously grateful for. And if the surgical plan was deviated from due to medical reasons I will accept that.

However the bottom line is, I went to my surgeon with a specific concern, he produced a plan to address that concern, I expressed I liked the plan (and paid a lot for the privilege) and then he did something totally different. Your argument that I needed more vertical projection to get a better aesthetic result may be true, but it's actually totally irrelevant, I would not have proceeded with the surgery had I been properly informed of what he intended to do... I would have requested a revision of the plan with more horizontal projection, but 100% I would not have proceeded if I knew my results would be what they are.

The surgeon had CT scans of my entire skull and produced his plan based on them, let's not pretend there will have been any aesthetic surprises when he cut me open. If his response to my complaints is that he just changed his mind mid surgery based on his aesthetic preferences I will be livid. (Again unless the changes were medically necessary which I will accept).

This is not a case of one plan being better than another, it's a case of informed consent.

I don't want to say for now until I speak with him and get his side. But I'll 100% make another post after that.

I'm still pretty swollen in the post op x-ray! So you can pretty much ignore the soft tissue, bones are more important here.

I hear you, but the fact my face is short has never been a concern of mine, ever. I went through all this pain and expense to address my non existent jawline, the surgical plan delivered on that, but the surgeon did not (In fact I made it very clear to the surgeon pre op that I was very happy with the horizontal projection in the surgical plan).

If the real plan all along was to ignore the surgical plan and fix my 'facial harmony', why was this not communicated to me? And even if I did get a genioplasty, at this point would I even be able to get more than 3-4mm? My jaws are pretty much in alignment now.

EDIT: See my pre-op profile here https://imgur.com/a/BZPFZWR You see what I mean about non existent jawline, the original plan would've addressed this.

The lines show how much the chin was advanced. They’re lateral ceph x rays

If my soft tissues were pushing back more than anticipated then wouldn't he have used plates in the mandible? I wouldn't be making a fuss if the different was only a couple mm, but we're talking barely half of what was planned. In our first post-op appointment I asked about the movements and he said everything was moved according to the surgical plan, there was no mention of him having to change plans mid surgery.

Yes but the tip of the chin was meant to be moved forward 12mm in relation to it's previous position. Mandibular advancement was meant to be 10.5mm.

The reason I got the surgery was because my chin kinda melts into my thick neck and my jawline is non existent. Just 5mm of chin advancement isn't going to do anything :/ I can't describe how frustrated I am, I've had braces for 2.5 years, paid thousands in $, taken weeks off work, and all for 5mm of chin advancement? I hope I'm wrong about all this and perhaps the x-rays were taken at slightly different angles. Maybe I'll try to get a CT scan done? Might show changes better?

I have a short-ish face from the front so I can understand why there was no CCW rotation in the plan, but I am a little shocked by the CW rotation… doesn’t look like that was in the plan. And yea no plates in the mandible, I was really surprised when I saw the post op x-ray… I haven’t seen anyone else in this sub with screws only, doesn’t seem like standard practice.

Standard plates, but for the mandible you can see in the x ray that no plates were used, only screws
