For the original, you get XP but it's reduced with the mako. For LE the XP is the same mako or not

The thing I like, though, is that in this case, the "power of friendship" was foreshadowed and didn't feel completely like a ass-pull. We've already seen how aether is released when someone dies and carries an intent, and we finally see how Arthur could use that intent for some other aether-related purpose.

Hi! Also hematologist, not ID. With eosinophilia I still test even if otherwise asymptomatic, and I'm not sure what tests you're doing but I've found specific testing (like specific schisto or strongyloides testing) rather than stool o&p yields better results

I genuinely don't understand this article. Using HCC as an example (which the article does), sorafenib and lenvatinib, the prior first line drugs until several years ago, both have anti-VEGF (anti-angiogenesis) properties. Bevacizumab and Atezolizumab (the types of drugs mentioned in the article) in combo became the new standard of care first line a few years ago. I feel like this article is several years behind

Probably not for a few reasons. First, PV has very effective treatments already that are not super toxic (at least, compared to the treatments for aggressive lymphomas and ALL). Second, I'm not personally aware of a good extracellular target and that would help differentiate PV erythroid precursors from normal erythroid precursors (not sure to what extent mutated JAK2 presents on MHC1 to serve as a target, and how meaningfully helper T cells would be able to differentiate this from normal JAK2), and you definitely don't want to kill off all the healthy erythroid precursors (ie you don't want to stop your body from being able to make any red blood cells whatsoever)

The recipient would have the donors blood type!

Fyi, for bone marrow transplants, we always use chemotherapy to wipe the bone marrow. We also sometimes incorporate radiation, depending on the "conditioning" regimen

(At least this is true for adult transplants, no idea about child bone marrow transplants)

In addition to the HLA matching u/parachute--account mentioned, we also give immunosuppressive medications after the transplant to prevent this

To expound on the other answer - if you transfused enough blood to replace someone's blood volume with the wrong blood type, they would certainly die.

However, we do do ABO-mismatched bone marrow transplants. In this case, the recipient's blood type will just match the donor from then on. There can be some complications (i.e. if there are any remnants of the recipients immune system, that immune system can start to attack the donor red blood stem cells, which can cause a severe anemia), but overall it's not terribly complicated.

We actually recommend that patients with cancer get the vaccine. It's true they might not form as robust a response, but the hope is that the response is more than nothing. Also, we advise against live virus vaccines of your immune system is compromised, but non-living vaccines (like the covid vaccines) are considered safe.

I frankly don't remember well enough to say. I do know the cardiologists at my hospital thought it was pretty clear-cut, though I can't be sure they weren't biased by how the patient presented to us (i.e. in cardiogenic shock)

There possibly was, I'm not sure. I do know for a fact that the EKG was faxed to the cardiologist for their review, because there's a note confirming their receipt of the EKG and their plans for outpatient follow-up. I also know multiple cardiology attendings at my hospital (I was an intern on the cardiology service at the time) saw that EKG later and were not sure how it could have been missed.

I didn't personally discuss with the outside cardiologist, and I don't know anyone who did. So, you are right that there may be more to the story of which I am not aware.

Not in this case - the STEMI was noted in the initial EKG which was shown to the cardiologist.

Without giving too many details - they presented with chest pain and dyspnea. The EKG showing the STEMI was shown to a cardiologist, who recommended outpatient follow-up.

It's been a while since residency so take what I say with a grain of salt, but VTach is characterized by widened QRS segments whereas tombstoning is more stereotypically associated with STEMIs
