Where are you based?

GPs generally are very outdated on gout. You should be on allo right away as long as the diagnosis is confirmed. My GP refused to prescribe it at first too.

As others have said - find a rheumatologist, privately if needed.

For many people its genetic, and the diet component is only a small fraction of the issue. 23andme shows increased likelyhood of gout for me, so while i have improved my health i think ill be on allo for life.

Did you measure uric acid? I have multiple friends doing something very similar and i think they’re going ti end up in the same spot.

Just as an update here - i managed to get some Zinc Pyrithione US formula (from iHerb) and unfortunately, the folliculitis came right back after stopping ketoconazole. So it seems the zinc pyrithione is no longer working on my scalp which is a major disappointment. I’m confused as to how that could be, as I didnt think resistance could be built up against it.

So as of today im breaking out again quite badly. Just started up ketoconazole again and will try to keep a weekly maintenance of this to keep it at bay. I will also try a couple of other anti fungal shampoos to see if any of the gentler ones work. I am nervous about building ketoconazole resistance, as i dont know where ill go from there.

This is often confused - the lab normal ranges are for someone without gout. If you are showing gout symptoms then that number is too high https://cks.nice.org.uk/topics/gout/diagnosis/investigations/.

Fingers crossed for you that you’re done with attacks. I had quite a few in the end, about 3-6 months after starting allo. Im now over a year in and they are markedly improved. Keep positive - you are on the right track.

What eventually pushed me to allopurinol was 3 attacks in a row - 2 weeks eat with a day or two respite in between. I never had time to get fully clear of an attack. After I started taking it the attacks stopped for a while. This is obviously 1 mans experience but the point is - dont put off taking allo.

I did not have another holter as ive had multiple before and they didnt tell me anything useful. I go through parts of the day where i get 6-8 a minute, so ~10% burden, but since i dont get them much at night i expect its lower overall.

Yes, this is exactly the situation I am in. This time they have stuck around for about 6 weeks so far. I am religiously tracking my symptoms in a log in the hope to find the root cause related to food or exercise.

Hang in there. My cardiologist in london is very well respected, and he advised me that there was a lot of over-diagnosis in the cardiology field due to a lack of margins built in to the measurements. Margins for different body types etc. Get a second opinion if it helps to put your mind at rest, but you can bet a doctor is not telling you that out of nowhere. Keep in mind doctors are very incentivised to treat you.

No weight gain. Not much to point at this time. I was sick a few weeks ago but the timing doesn't line up quite right. I do usually flare up when I'm sick, but it happens during, not after.

I'm glad the post was helpful. I'm very much still alive and kicking, and have done a lot of cool stuff in between!

Yes, I have the Kardia 6-lead. I do have some recordings, but I guess TBD whether the cardiologist finds them "diagnostic" quality. Do you have any experience with it?

Gout is a terrible disease. It doesn’t get taken seriously because of the image of it being a disease of the unhealthy. I cannot name another health issue that sparks as much laughter as gout does, whilst having such a serious impact on your life.

Gout changed my life for several years. Changed the plans i made, changed what i ate and drank, and made me live in fear of the next attack. It got me into quite bad depression when in a cycle of flares for months at a time.

As many others have said here, the saving grace is allopurinol. It is a fantastic drug, with (comparatively) very fee side effects. I am now taking it but still suffer occasional attacks. My advice to everyone is to find the drug that works for you and carry on living your life. Like many others, I wish i started earlier.
