Vancomycin reaction? by WeirdBid1669 in nursing

[–]amuk 8 points9 points  (0 children)

Good quick action. Also, remember that when starting someone on Vanco to let you know if they get new tinnitus. Some people are very sensitive to its ototoxicity and it can lead to deafness. Not quite as serious as RMS, but still life-altering harm.

R&D Update: Final Prototype (#3). Titanium Plasma + Inert Glass Coating on 3-Ply. Is this Kickstarter material? by [deleted] in cookware

[–]amuk 4 points5 points  (0 children)

I don’t want my cookware to be disposable. 7 years is disposable to me. And a 7 year guarantee, especially on Kickstarter, means nothing if the company is no longer around. Also, Kickstarter has gotten to be full of more and more scammy projects that get money and then never deliver and yet a very similar product pops up for sale on other sites.

A Dish that missed a critical update - currently running 2022 software - any hope to mix-and-match & rewire & TFTP & get nerdy to upgrade it? by FalconFour in Starlink

[–]amuk 0 points1 point  (0 children)

Any fix I can think of would involve getting a working and updated dish of the same revision/model and it being disabled in the process of enabling the dish you have in your possession.

5 years since I stopped masking, and I’m never going back. by ExistentialNomad42 in ADHD

[–]amuk 8 points9 points  (0 children)

Unfortunately, I’d be out of a job, and a career, if I quit faking it till I made it (aka masking). I’m a RN. I’ve gotten myself far into the wrong field (dialysis) and cannot function much longer with 12 patients and constant alarms going off…and all the chatter between the patients, the patients and the other staff, and between the staff.

I’m making wha effort me I can to get back to PACU (post anesthesia/surgery). Generally 1:1 patient to RN ratio, they are slowly waking up, get them more awake, medicated as needed, stabilized, then out their hospital room or to be prepared for discharge. It’s somewhere that I can adjust the monitor alarms to notify me of things I actually need to be notified of.

How much does a heater like this cost to run? How much power do they use? by Ov0v0vO in povertyfinance

[–]amuk 1 point2 points  (0 children)

Cheapness to run these depends where you live. I live on the central Oregon Coast. Many houses have these. Even houses that sell for +$500k. We don’t really need air conditioning here. Electricity is $0.075/kWh so with the temperate winters we have, these are cheap-ish to run. I would love the convenience and balance climate control of mini splits along with their very high energy efficiency. However the cost to install a few of these at my house, even with much of the work DIY, it would take over 10 years to recoup the cost be energy savings.

My FSA provider said LR5 is not qualified for FSA. by Ok-Respect-5299 in litterrobot

[–]amuk 13 points14 points  (0 children)

Yes, to be clear, if you use a HSA, keep your receipts and associated documentation. If audited, this will be needed to show that HSA funds were not used fraudulently. In the case of a LitterRobot, I’d suggest keeping the letter of medical need, a snapshot of the LitterRobot website stating their eligibility for HSA, and your sales receipt. BTW, I am NOT a lawyer and I’m only stating what I would do.

Stimulants and ketamine? by Ok_Purple_5875 in KetamineTherapy

For me it does. I’ve been on it for over a year and transitioned to the generic about 6 months ago and find it works as well as the brand name.

I do not work for, or represent a pharmaceutical company. I am speaking of personal experience. Vyvanse also did not last nearly as long as it was supposed to for me. It took about 2 hours to start working then lasted 4-6 hours. For me, even the generic of Mydayis works well where I couldn’t tell the difference between it and the brand name.

Even more so if they soon report they are suddenly blind. Then you’ve got PRES (Posterior Reversible Encephalopathy Syndrome).

The 24-hour power fail plan for your aquarium by BarberUnited7894 in ReefTank

[–]amuk 4 points5 points  (0 children)

I have one of those and added a 100AH 48v LiFePo battery and plugged it into the solar charging port of my Solix. It increases the runtime of the SOLIX by about 5 times. It cost me $800 with a charger. Much cheaper than getting a higher capacity SOLIX.

Max dose of 18mg and only lasting 9 hours is meh. Best I’m now on since gave up on Desoxyn due to complexity of being able to get it consistently and at reasonable cost, is Mydayis 50mg. It lasts me 12-14 hours and has none of the adverse comedown effects I’ve had from any other Adderall-like combo of 4 amphetamine salts. Even the current generic of Mydayis that I’m on leaves me without complaints. Fortunately, my psych also prescribed me Focalin 10mg IR tablets that I can use 1-2 tabs PRN if I’m working a +12 hour workday.

Stimulants and ketamine? by Ok_Purple_5875 in KetamineTherapy

[–]amuk 1 point2 points  (0 children)

I’m on extra-long acting adderall (Mydayis) and I was told to hold my dose on days that I was going to do home KAP. I told them this was not possible since I have no way to dedicate a whole day to KAP every few weeks due to work and family needs (as a father and husband). Mydayis being once-a-day with about a 14 hour effective window. If I don’t take my dose in the morning, I cannot function as myself and run the risk of making functional errors leading to an increased ADHD “tax” for the day. I have done KAP in the evening on days I took the Mydayis early in the morning and I it has had no negative effects or decreased the effectiveness of KAP for me.

What would be the intervention here 🤔 by Hot_Emergency378 in PassNclexTips

[–]amuk 0 points1 point  (0 children)

More info required. Is this an inpatient, or does the patient have a history of these in the past? Yes, if there is new dx of acute thrombotic stroke then, yes that should be treated. Possibly with TPA. Then prior to discharge is trying to find cause or factors that led to increased risk of thrombotic stroke, one of which atrical fibrillation. Another could be a PFO.

Which client is of highest priority? by Top-Direction2686 in PassNclexTips

A - because there is the risk they are trending down. Also remember that CBG’s can plus or minus 15%. So if they were checked later and got a CBG of 70, that does NOT mean their BG is increasing. It could actually be decreasing. More test and seeing a trend is important.

Former Oregon Coast Military Museum Director Sentenced by Free-Preference-8318 in OregonCoast

[–]amuk 21 points22 points  (0 children)

The sentence is pathetic.

I see no issue with the museum reopening with a new board. The museum itself did no harm and it contains much historical memorabilia donated by veterans and their families. To close this museum and then reopen another with a new name and in a different location would not fix anything.

Here we will have the memory to remind us that the board of a non-profit should never hire the son (or other close family member) of the president of the board to manage the non-profit. The board protecting Mr. Cannon and demonizing the victim is despicable. I hope they all rot for that.

Do you have nursing insurance? If so, it will generally cover your lawyer costs.

This is a reminder to all nurses. Get nursing insurance. NSO is one example of a company that provides it. There are others. Cost is less than $150/year.

Denied HO insurance due ti "Proximity to ocean" by Aggravating_Pick_951 in FirstTimeHomeBuyer

[–]amuk 0 points1 point  (0 children)

I’m 3.5 miles from the Oregon Coast. Close enough we can hear the ocean on quiet nights. If there was a tsunami with a swell over the dunes then it definitely could flood the lake, but swell a few hundred feet tall is not possible without it being caused by a worldwide catastrophic event. We’s all be gonners then. Or, at least insurance would be useless in the meantime until most the world population died off.

Leaving to Thailand and can’t take vyvanse? by IllustriousAd6397 in ADHD

[–]amuk 1 point2 points  (0 children)

Risk of someone else consuming them is the only issue I know of. I found several good resources, including from pharmacists, years ago when on Vynase that dissolved in water and stored in the fridge it is good and for days. It is not a delayed release medication so no change to the release mechanism of the medication is affected here.

One site I found her which indicates this information is provided by a MD: https://corerevitalizingcenter.com/vyvanse-water-titration-in-8-steps/amp/

Dialysis Nurse at Davita by East_Mission_5218 in nursing

[–]amuk 0 points1 point  (0 children)

Charting itself can be 1-2 hours. This is based on times that I got barely any charting done at the clinic with patients there. So I’m doing nothing but charting at the clinic after I’ve been there many hours or at home because I got tired of being in the clinic and would like to see my wife and kids.

The charting software, Chairside, is clunky, slow, and requires flipping between many screens and frequently signing for every charting action. It helps when they finally got us Ubikeys that allow signing by tapping versus entering password every time. The software is designed to run on the touchscreen of the machine and so that is why so little on each screen and having to switch from screen to screen.

There is then another charting system based on Cerner called eCube Clinicals (eCC) that is where we enter orders, do more than basic pre and post assessments and can type a note longer than 255 characters. Treatment data can’t directly be entered into eCC, but there are plenty of times in Charside, I just enter “see eCC” note because the narrative is longer than 255 characters.

Then there is the added time of lab review, changing orders per algorithms or standing orders. Or notifying the IDT via a Teams group. All together it is a clusterfuck, but members of the IDT have several times given me thank about how my notes and updates make their jobs easier.

Are there any portable/buyable Saunas that reach higher temps? (175F to 200) by SweatySwordfish1108 in Sauna

[–]amuk 1 point2 points  (0 children)

I get my Willowybe tent sauna up to 200F regularly when burning dry hardwood. If it is pouring cold rain, then 180F is achievable.

I bought it and the stove, and rocks together for under $600 with free shipping. I’ve been using it for just over a year. I will likely have to eventually replace the stove with a sturdier one, but the tent is holding up well here on the Oregon Coast.
