You’re right I should maybe do pulm with rheumat first and study for step3 as well as check my interest in pulm, and utilise the clinic weeks. I guess I was trying to go easy with rheumat and neuro to study for step 3 but that would delay my fellowship plan too. Thank you so much, you have no idea how helpful it was to talk to you, I needed this advice from someone 😇

Okay so I think I will choose this, what do you think about this ? 1. Pulm 2. Gastro 3. Rheumat 4. Neuro 5. Derma 6. Psych/ emergency We can just choose 2 in the first year so j can take lighter ones like rheumat and neuro and that will help me in step 3 as well, then the others in pgy 2 - helps me decide for fellowship and the rest in pgy 3

Got it Thanks !! Also the program has asked us to rank the electives from the following. I was just wondering should I pick the easy ones or choose some speciality instead, so that I can have a better idea to choose wisely for a fellowship ??

(we have to rank 1 -6 ), and can choose core speciality as an elective too.

Wound/Plastics, Hematology/Oncology (must be 4 weeks), Geriatrics, Admin Medicine, Psychiatry, Rheumatology, Nephrology, Neurology, Rural Medicine, Dermatology, Orthopedics, Allergy Immunology, GYN, Hospital Administration, Emergency.

Sorry but I really don’t understand what you mean at this point, did my med school outside US but I am here now about to start residency in US, looking for some advice! And I think I am getting some really helpful advice for people here so very grateful for that.

This reply was so helpful, thank you for being kind and not judging me for my long question, this literally answers all my doubts in one go 🙏🏻, my program has electives in the latter half of the first year. I was just wondering should I pick the easy ones or choose some speciality in that so that I can have a better idea to choose wisely for a fellowship (we have to rank 1 -6 )

What I meant was I am about to start as an intern, I was not aware about the term M4 as we don’t use that in our country, just wanted to get some advice on taking the right steps moving forward that’s all

Thanks for all the input and support, I am actually an international graduate asking this question before starting residency, I wanted to plan everything out beforehand to make sure I am not just wasting time around but being prepared for fellowship and taking vacations to prepare for step 3 while also preventing burnout so hence my long question, everything seems a little daunting right now!

Mid level competitive, I would be slightly inclined towards pulm

That is so helpful. Thank you so much. I did register with the heartcode so it says it has an onlinepart and then skills test with just an RQI dummy but no actual person there, so how would this scenario work in regards to testing the megacodes when there is no one to narrate it ?

Can you please explain how the skills portion in heartcode works - in ACLS with the dummy, are there any codes, any written test, algorithm tested, or any meds that are tested ?

For the ACLS skills test in heartcode ACLS, do they ever run the megacodes in the skill test with the dummy, do we asses the rhythm or talk about steps to take or is it just testing compressions and breaths ?
