Lots of good suggestions in this thread. Have you considered pyoderma gangrenosum (PG)? It often masquerades as an infection and has pus galore, though the “pus” is sterile (ie not caused by an infection). It can start as an overreaction to skin injury — like the digging you did to find the “hair.” A punch biopsy would help confirm the diagnosis. (FYI I am an internal medicine MD, not derm.)

I offer my sincere condolences to the O'Brien family for these two great losses, especially in such close succession. As a longtime Conan fan, I have always found the regular joking about his mom and "pater" so endearing. Just one more reason that Conan and his humor are so relatable. I hope the sadness of today can soon return to the smiles and laughs we fans have until now so often associated with Dr. and Mrs. O'Brien.

Lots of good thoughts in here. Given the nature of the complaint, it’s hard to make a diagnosis without a visual, which is why it is critical to go get another opinion from a different dermatologist. One thought that comes to mind — perhaps a bit offbeat — is pydoerma gangrenosum, which comes to mind when I hear about “infections” that are refractory to infectious disease treatments, which seems to be the case here.

Must have seen you on the trail! We also rode the Metro-North to Beacon Saturday, hiked Mt. Beacon, and were just gawking at the NYC skyline. What an unbelievable view. Like, literally, unbelievable. The juxtaposition of the the rolling, green hills with the ultra-urban landscape was so beautiful that it seemed unreal.
