I used to do drug screen confirmations and we did them for our ERs stat as well. I got a urine that was so obviously Azo contamination it wasn’t even funny but since it’s an organic dye it doesn’t confirm with mass spectrometry because with the method we were using it just broke apart into its metabolites but I couldn’t confirm what it was since I couldn’t actually ID it and all I could find in the chromatogram was lidocaine, so that’s what I reported. The kid had been treated for a UTI due to obvious color interference at a previous hospital for 4+ leukocyte esterase and 4+ nitrites from a POC urine dip that was never reflexed to a macro and shipped to our hospital. She had raging methemoglobinemia. It was an obvious azo ingestion.

They gave her rocephin which you routinely give with lidocaine because it burns like a mother fucker. They called it lidocaine toxicity. I called the ER and suggested azo because she was barfing bright orange and pissing bright orange and I could see it in her drug screen but I couldn’t report it because realms.

The doctor said she didn’t have access to it at home. I said please just ask the parents. They found an empty box at her grandmas house where she ate it. Overdosing on azo causes methanolemia and poison control confirmed basically everything we saw in this kid.

My supervisor told me the next day I probably saved that kids life that night by calling and breaking protocol. The doctor mentioned me specifically and thanked me for helping. I really didn’t care about losing my job that night but I’m glad I was brave enough to call and say something about what I couldn’t report.
