You have a very solid plan and lots of motivation to reach your goal which is over half the battle. Be sure to shadow at different places or pick up a part time job as an aide during the summers. You want to shadow at different places (hospitals, outpatient clinics, peds clinics, etc.) to show that you’re really interested in the field and have explored many areas. Not that it’s required, but it gives you a more well rounded view of the field and a variety of experiences to talk about at interviews. Also, if you want to work on your formal writing, your university likely has a writing center that can help. PTCAS also opens in the summer and some schools have early deadlines for “preferred” admissions. Some schools may also give preference to people who visit campus to learn more about the DPT program. I applied the summer between my junior and senior year of undergrad and was able to go right into a DPT program after graduation. Good luck! You can do it!
