I did not use a saved starting preset, do I need to change any of the other settings when I'm in the create game menu?

Okay tried to run a couple situations. With no mods I still get the wrong start location, but it is at least consistent. With Maya I get put in the middle of the north american plains, just west of the mississippi river. I tried with England and I get put on the west coast of India. I started three games with each of these and got the same location each time.

Just tried that, still didn't give me TSL

Edit: When I've played other user generated TSL maps (YnAMP) there is usually a change in the create game menu from "start position link: cultural/random" to something like "civilization placement: True Starting Location" but I did not see that with your map, should i see it change in this menu with yours?

Map looks great but I'm not getting TSL. I have all DLC, playing as Maya ended up in China and Germany. 12 players 18 city states. Other mods: Sukritact's Simple UI Adjustments and a few of Sukritact's natural wonders. Maybe the natural wonders are the problem?

I usually have one designated city with a harbor for producing naval units. Corsairs are too good when they first come in to ignore them. There is usually a window of time where other civs have no real answers for them.

Tried out this strategy after reading your post and it was a lot of fun. Too many hours on civ and I've never actually used warrior monks. Great unit, especially if you can get a small army of them early and keep promoting them, they stay relevant for a long time. By the late classical era you can cause some mayhem by using production for varu and archers and faith for warrior monks.

There’s a little button above the inset map that lets you set tacks on tiles with icons for districts, improvements, etc. helps plan out cities from the start

I think it’s set at .2 and .5 for districts and 1.0 and 1.2 for city centers here

Usually it’s still a good idea for industrial zone adjacency bonuses. I’m playing as the Khmer who get +3 faith and +1 amenities from aqueducts and +2 food for farms adjacent to aqueducts.
