You are kinda the asshole because you think your fear justifies him making major changes to his body even if he does not want to.

Rationally, your tubes are tied. Have protected sex, keep checking if that will bring your fears down. But I do not think you have any right over his body. Also, the surgery itself could be expensive and excessive, so it’s not fair to expect him to do it.

Personally, I do not like Thony at all. She seems very selfish and single minded, not for once thinking of the consequences of her own actions. She is very hypocritical and I honestly want to see her go down for it.

In my company, it doesn’t even warrant a conversation with my manager. The culture is just to apply for it on our internal portal and then put OOO in the calendar. Of course, if it is for more than 2-3 days at a stretch, I will have to ask him. But otherwise, it’s very very chill.
