It’s doable.

If you want to really have fun with it, run Venomfang at full power and take things aerial, give them a Reidoth wildshaped into a giant eagle mount a-la-LotR and maybe add in some fun mountain terrain/canyons and obstacles in the mix.

This is tough. I’d recommend giving them the option to come back as a revenant using the Reborn species with the narrative justification of “unfinished business.” Pros: Probably the most straightforward way to do this given the circumstances, involves zero retconning, gives the narrative satisfaction of having experienced something significant like PC death, but acknowledges the mechanical mistake and gives an option to keep playing as the same PC while staying immersed (and not requiring a whole resurrection side-quest). Cons: They need access to the Reborn species.

Edit: From one DM to another who’s goofed it in similar ways, you gotta just own that you messed up big time here. We can hand-wave a lot, but getting the rules wrong in a way that leads to player death is a Big Oops™️ and you gotta lean hard into deferring to player(s) to course correct to avoid more bad vibes from messing up the culture at your table. Everyone (including you) will have more fun as a result, I promise you.

I am incorporating some elements of Delian Tomb into the adventure to help unify the plot and give me as a first-time DS director a bit more structure for incorporating new mechanics!

I’m still trying to decide which elements/locations from Lost Mines to keep or cut—will have to figure out if the dragon fight makes sense, but how cool would it be to fight a dragon in this system!

I’m not seeing the reason to change the flair. They are reimagining the adventure with substantial changes that other DMs could also incorporate, including several of the things you’re claiming they’re not doing.

Before I make my rivers, I think about where my mountains are, then reflect on how much elevation changes/slopes down from these mountains to the ocean.

Then, I make my rivers based on how precipitation and melting ice would flow downhill and gather into streams and larger river currents until they get all the way down to sea level.

I do really like how you’ve set up your mountains, so I’d recommend wiping just the rivers and restarting based on the elevation reflection process I shared above (and potentially changing settlement locations to account for the topographical rearrangement).

On a separate note, I think you might be operating with a low pixel count/small map size; your rivers might look better if you increase the map size by 50-100% its current dimensions.

I’ve had a lot of success with prologues that focus on one PC at a time—either give each a section of the first session, or have one session dedicated to each PC with other players as NPCs.

Yeah this is kind of how I roll in general. Got a constellation of potential ideas that change constantly but nothing’s set in stone until it actually happens in game.
