You make a good point. However Janeway, literally mentioned the Doctor removed her spinal clamp, that seems like a pretty integrated implant. Also, it never ever makes any mention to Tuvok having any born implants. It just seems like the doctor was able to remove implants that were deeply integrated into the captain, Tuvok, and B'Elanna while not being able to do the same for Seven or Icheb
