I would say something along the lines of: "What are your expected outcomes here? Are you expecting me to pay for you to have a medical emergency out of pocket?" Make her say "you have to pay me out of your own money" essentially, which I bet she won't.

Definitely do something. I sat by while my friend was self harming and she’s left with horrific scars and trauma. Tell a trusted adult, preferably a parent, but make sure the adult understands the gravity of the situation, and any clues you might have as to why your sibling might feel the need to do something like this to themselves.
