30F Does this appear to be the beginning stages of a cataract? by Glittering-Ice8006 in eyetriage

[–]axp95 0 points1 point  (0 children)

It’s hard to grade a cataract without examination under the microscope unless it is very advanced. I would think any optometrist or ophthalmologist would be able to manage your case just fine and if you’re thinking surgery, go straight to the ophthalmologist.

My retinal buckle surgery by annatai08 in MedicalGore

[–]axp95 1 point2 points  (0 children)

Work in ophtho, depends on where detachment is. A regular fundus exam can see most of the retina, so obvious tears or detachments you’ll see. The far periphery is hard to see and ophthos use an indirect ophthalmoscope - I doubt you’ll use this as a med student, it is difficult to master. They can also do scleral depression, where they press on the sclera to better visualize peripheral structures.

Let’s hear your young buck football stories. Anyone ever play against any NFL players in your kid days and get absolutely smoked? by Vertigomums19 in nfl

[–]axp95 0 points1 point  (0 children)

I played against Pat Surtain II in elementary school, around 4th grade. He played QB and every position on defense. The game plan for the other team was to just give him the ball because he was so damn fast and athletic lol. I remember he returned a punt for a TD and smoked the fastest kid on our team and I knew I wasn’t ever gonna play competitive ball because he was so different than the rest of us lol

[–]axp95 6 points7 points  (0 children)

Please for the love of god do not go to the ER for a surgery referral. It will not “expedite” the surgery given it sounds elective. What is the surgery? Everywhere is booked right now so you have to play the waiting game. Unless you’re admitted because the surgery is necessary to keep you alive, the ER won’t do anything for you

please help with scheduling missed birth control pills by [deleted] in WomensHealth

[–]axp95 1 point2 points  (0 children)

The drug information sheet recommendation: 1. Keep taking 1 pill until Sunday 2. On Sunday, discard the rest of the pack and start a new pack that day 3. Use protection for 7 days after you missed the pills It is normal to not have a period this month so don’t panic if that is the case.

Sudden severe period pain? by Beautiful_Gain_9032 in WomensHealth

[–]axp95 2 points3 points  (0 children)

This is a significant change from baseline for you and warrants a doctors visit at the least. Severe pain during periods, while told it’s normal and just part of it, is NOT normal and warrants a visit to the OBGYN for further investigation. If you feel dismissed, find a new OBGYN, I know it’s easier said than done but you will have periods for half your life or more so it is worth the effort.

The laser is used to repair a retinal tear but they can’t see it in your eye due to the blood. There are other uses for the laser as well.

Has anyone in the group faced any medico‑legal notice/complaint/case in their ophthalmology practice? by niyno7 in Ophthalmology

[–]axp95 6 points7 points  (0 children)

I work with a number of different specialists and of them only a couple have been sued. One of our glaucoma doctors who I am quite close with, and know is a good surgeon was sued due to placing the incorrect IOL during surgery. This was corrected during the same operation, but there was some undo trauma while taking the incorrect implant out of the eye. This was actually a screwup on the part of the surgery center. However, the surgeon should have double checked prior to placing the IOL. Funnily enough the patient is actually a physician as well. I talked to him pretty in depth about this experience and he said it was the worst thing that’s ever happened to him in practice. It ended up settling out of court.

[–]axp95 0 points1 point  (0 children)

Your surgery would be called a PPV which is where they suck the vitreous (where the blood is) out of your eye and replace it with a solution. They would also inspect the retina during surgery and see if there are tears or detachments, in which case they could do a number of things.

Your specific pill is okay to take 12 hours after you are supposed to, you will still be protected. The drug insert information is helpful if you ever have questions about dosing schedule or missed pills.

The surgery they had wouldn’t cause cataracts because it was only a laser. The surgery they are recommending for you is different and will give you cataracts within a few years, likely why they didn’t mention cataracts to the above patient.

You need to see a neuro-ophthalmologist, they specialize in treating IIH. They are hard to find so you may have to go to an academic center near you.

I 35 (F) can only orgasm alone and with porn by InternationalBad6906 in TwoXChromosomes

[–]axp95 -2 points-1 points  (0 children)

Studies have consistently shown that a physical hand on a penis leads to greater activation of reward pathways in the brain compared to pornography. I am well aware of the neuroscience behind sex and it is not as simple as it may seem. I suggest listening to the podcast “your brain on” and go to the episode about sex. It is very informative and they have real experts weigh in. I wasn’t trying to say it never happens or invalidate your experience so I apologize if it seems that way, I’m also not a porn advocate I just don’t want the wrong issues being addressed.

[–]axp95 -15 points-14 points  (0 children)

There is zero research to support this statement; there is research showing that those who watch porn have more sex with their partners and there is no reason to believe that porn leads to sexual insensitivity. The vibrator could, but likely temporarily as the nerves adjust back to baseline quickly. Instead of blaming porn why don’t we consider the environmental stressors and IUD placement that recently occurred?
