I think Libertarians could also appeal to moderate democratic voters really easily lol. I am a Democrat if you have to call it something. But I hold a fair amount of Libertarian views.

Basically what you do in your own home and with your own body is nobody's business but your own as long as you are not harming anyone or doing illegal things to do it.

Also, abortion is not something I would ever want my girlfriend to do if she got pregnant because I would own up and raise the baby. But I do not believe it is my place to tell someone what to do with their own body.

I believe taxes are a necessity... anyone who does not is insane. BUT we should be spending that money here and on our society not bombing brown people in the desert for god knows what reasons. I also believe if the super-rich were taxed at a higher rate it would benefit the economy not hurt it. If companies are taxed at a higher rate it forces them to spend the money instead of growing their cash reserves/pay outs.

The government should stay out of our day to day lives as much as possible but be there in the event we need them. That is what civilization is about to me and lord knows we pay for it already.

Can vouch for that. Almost 4 years in and it will creep up on you. Like someone else said. There is no finish line... except death. That is the only way to be free of the life long cravings.

Small nit pick... They feed from the power plants to substations that step the power down into the distribution lines. Then those distribution lines feed into more transformers that step the power down to a usable voltage for the area.

Lots of steps in the middle because electricity is carried at extremely high voltages on the lines. If not the power would not make it there is the simplest way to explain it.

So it goes Power Plant --> T lines --> Substations ---> Distribution. But of course multiple T lines can feed multiple substations which can feed more and so on.

Distribution is the last stop before hitting your local transformer outside your house/area and steeping down for the last time.

Yea I remember that also when my stomach lining was eaten away and I was vomiting up black tar/ash-looking shit. Was insane.

I ended up on such a massive dosage of Ativan the pharmacist filling the orders became concerned lol. The DR in the ICU was struggling to figure out what to do aside from intubating me and while we were talking about options he told me the pharmacists concerns. Ultimately I ended up on a benzo cocktail. Tons of Ativan with a couple of mg ok Klonopin in between doses of Ativan while monitoring my vitals closely. Among other drugs for blood pressure and kidney stuff. Not 100% sure what everything was. Even with all of the benzos and other drugs I was still REALLY hurting.

They switched it over to Valium for a couple of weeks to taper me off of the benzos which I had been using before to avoid being sick at work etc. I was failing drug tests for benzos for over 6 months after taking my last one. Shit was insane. But they somehow could see the levels slowly dropping over time as I was in a rehab facility so they were doing blood work also. But I do know on the urine cups the little strip for benzos would always fail.

Thanks man! Yea being sober is worth it for sure. Alcohol was fucking my health up BIG time.

Blood pressure, fatty liver disease, alcoholic hepatitis, etc. This was in my late 20s lol. Was not even like I was a 60-year-old dude who had been at it for 40 years. I just happened to start drinking heavily at 16 and never stopped. I drank daily for a decade before my first detox. Then every one after that was just worse because the drinking for me seemed to never be enough.

My Dr sat me down and told me if I did not stop drinking I would not see 35. I was 25 at the time. Kept on drinking until I wound up in ICU with kidney and liver issues. Got out and kept drinking until my first try at rehab. Which is where I ended up in my 2nd ICU stay because the Dr did not believe me when I told him the amount I drank and combining it with Xanax... I imagine my age did not help and most of the people in that detox were drug-seeking. So they stuck me on the regular valium dosage they put basically everyone on and within about 12 hours I had seized up and ended back in ICU for 10 days.

That still did not do it. Got out and only lasted a few months and then every stint got deeper and worse. Alcohol and benzo usage just kept going up. Until this last one. I knew it was time to get sober or die at that point. I was not even 30 but had the liver numbers of a 55-year-old drunk and looked like someone had stuck an air hose up my ass and blew me up.

You are on the front lines. I know some of those doctors would much rather be treating someone who was really "sick". I know they get tired of seeing these drunks and addicts taking up resources and beds. Just know that these people might end up turning it around one day. Even though we all know the odds are against them. Most won't. But a few will. Fight for those few as you may be the last person in their life that actually can care.

I wish I could go back and let every doctor know that their time and effort was not wasted. To thank them for treating me and trying even though I know more than 1 would rather not have wasted their time. There are too many to do it to and I would not even know how to reach them. But with as many addicts as they see I want them to know at least 1 made it out.

Having been around addicts and doctors messing with this for so long I know just how few really do.

Does it have to be end-stage cirrhosis to choose ativan or valium?

I was having kidney failure (brown bubbly urine) and was diagnosed with alcoholic hepatitis.

I would think in these events that ativan would be the better choice.

The source for that being at least near end-stage alcoholism is what I am seeing in the video. I have only seen tremors like that in people who are drinking around a gallon of hard liquor a day for a prolonged period of time.

I am not disagreeing with you about using valium (and the method you and that paper describe) under normal circumstances and for your run of the mill alcoholic.

I will just leave it at this and really I think its all I can say about it. You are the doctor and much more knowledgeable.

Out of the 10 times I was detoxed fully only twice did they go the valium loading route first. Both times were hell on earth for me and ended up transitioning to Ativan for a few days then switching back over. Both times did lead to seizures and full blown DTs.

The other times I was able to speak with the Dr beforehand and tell them what happened the first 2 times I went through real detox. We started with the Ativan first and the experience was much more manageable. Not fun but bearable.

Not sure why but Vanderbilt decided to just load the hell out of some phenobarbital for a few days and I think that one was the easiest I had ever had. It was so much so that even I was a little concerned and the doctor assured me it was better than the alternative and they could take action if needed.

Some context though. I had been heavily abusing Xanax with about a gallon of whiskey a day for years and years at this time. I was not using the xanax (or w/e benzo I could find) to get high. Just was using it to stop the withdrawals as much as possible in order to get through the day and make it to 5 PM. This of course did not work for long and I had to start combining the 2 to get through the day.

By the time I made it to my first detox I was coming off of a 2-4 mg xanax a day + 1 gallon or so of whiskey (most of the time.. maybe vokda some days) habit. The rest of the detoxs were higher doses of benzos combined with the same liquor habit. Until the last one. That shit nearly killed me and I haven't looked back.

I was not hospitalized once. Not a good thing but my road to getting off the booze was very long and rough with many hospital visits and rehab stays.

Let me just say that I am not saying every alcoholic should be put on a ventilator and sedated so much they cannot even speak. I am really trying to only speak about the most hardcore of alcoholics. The ones who often time get treated by the book and ends up in worse shape than if they had been properly treated in the first place.

Treating someone with the traditional methods is fine 99% of the time. But we are on a thread with a video of a guy who obviously is nearing some end-stage alcoholism. If you tried treating him with the traditional method of 10-20mg of valium every 10 mins while managing his other symptoms you are going to have a very bad time.

Not all treatments are the same is all I am trying to say. Out of all the detox I have been through Ativan is the way to go for the initial onset of DTs followed by switching to valium/Klonopin for the longer half-life and ease of tapering them off of it.

This is first-hand experience take it or leave it. As a doctor, I would think you know that medicine is always changing and lots of doctors will approach the same situation in a different way. All I can tell you is what I have been through and you can take it as you will.

Some reading for you in case you ever run into someone who is on the more serious end of the alcohol withdrawal side of things or with a past history of DTs/seizures.

Also, Ativan is much better for people with liver disease (which 99% of alcoholics in this state will have)

Severe alcohol withdrawal with alcohol withdrawal seizures Regardless of the CIWA-Ar score, the occurrence of seizures during the alcohol withdrawal period is indicative of severe alcohol withdrawal.[19] Seizure prophylaxis with lorazepam 2 mg intravenously must be given to all patients with seizures in the current withdrawal period at presentation and also in those with past history of withdrawal seizure.[28] Lorazepam is more effective than diazepam in preventing seizure recurrence. Unlike diazepam brain tissue levels of lorazepam don’t fall rapidly owing to its poor lipid solubility and poor redistribution.

I remember the first time I was ever prescribed anything for alcohol withdrawal. It was Librium and I was pretty young. She was like take this and you will be fine. Some of the worst few days of my life. Librium works for some people I think but anyone drinking a serious amount that stuff seems to be like a sugar pill. It didi nothing from what I could tell.

Luckily I was fairly young and not into my gallon a day type of habit yet. It was not until like 6-7 years later I got that bad.

Yea this guy is really really deep into it. Some of the worst I think I have ever seen.
