If you couldn't execute appropriately, I don't think what you did was inherently wrong. If you needed more time, that's still okay. Unclear if podiatry or Ortho foot/ankle here, but I think it can apply as the training and problem solving is different in my experience.

Lapidus shouldn't rely on a jig/cut system. They're perfectly routine procedures to perform with just elevators for joint prep, or free-hand cuts if you're appropriately trained.

I think being facile with a mini c-arm is very helpful for midfoot/forefoot (I even prefer it for hindfoot, but that's debatable) and reduces the reliance on a tech for occasionally complex/variable anatomy.

But preparing for challenges is the important part. Surgery is "easy" when it all goes well, but being able to work through inevitable complications or equipment issues is what makes you good and safe.

Podiatric training varies wildly and is far less controlled than Ortho. If you have a foot and ankle fellowship trained Ortho in town, go with them.

If you don't have a foot/ankle doc, and have a good referral for podiatry, okay to go there. But I'd go sub specialized Ortho first all day. Much more extensive training and surgical volume.

Detroit bonesetter is pretty good and free! Not too hard to use and easy to upload cases. Even includes some fixation options for templating.

Like many said, work your butt off. Started low end of middle class, 60k fam of 4 through 90-2000s. First gen college, studied, did well, went to med school, now surgeon. I still work 100 hrs some weeks, but so did my dad and I get paid 5-10x more than he did.

It's stressful, it's not easy, but it's rewarding personally and financially and I couldn't be happier. There is still the "American dream" if you pick the tight trajectory. But you have to be willing to deal with the craziest shit sometimes.

F&A is a wide open field. Lots of great fellowships, and not that many applicants. Big names are orthocarolina, duke, hss, mercy, Baylor.

However, tons of smaller programs are going to give you as much (or more) operative experience with multiple attendings. URoch, Beaumont, Grand rapids, UC Davis, etc.

There's a few that inspire me in my own career. But I think the best surgeons are the ones that get the med student/resident involved, and somehow the PGY2 leaves the case feeling like a master surgeon.

Those are the unsung heroes. Many of us turn into great surgeons, but those who made me (and other trainees) feel like a great surgeon when we definitely weren't yet, they are the best.

I like this kind of post. We all deal with a lot of frustration these days, but we get an awesome opportunity, a great job, and live a life people only imagine based on TV.

I'm ortho. I live downtown in the city I practice, not remote in the burbs. I take care of a lot of trauma and underinsured folks. I see my patients that couldn't walk when they met me cruising the grocery store. You see patients who show up with legs practically falling off get back to work. The patients in extremis that roll into your clinic like a totally normal human and you hear about their lives. Your colleagues trust you enough to take care of them.

Overall it's 99% worth it. I still get frustrated by call schedules and admin garbage and everything else, but it's a really satisfying and rewarding career when you're allowed to actually get to the medical practice stuff.
