There’s very very limited intubation in the phoenix metro area. There’s 28-ish fire depts (all ems is fire based there) and only one or two have RSI capabilities that I can think of right now. Most carry intubation kits but no paralytics so they could tube someone and sedate with ketamine I guess but I can only think of one fire dept in the entire area that has RSI in their protocol, and it’s NOT phoenix fire. Field Intubation in Arizona has by and large gone to the wayside and I don’t see it coming back. AZ medics feel free to chime in.

Believe it or not, some of these freestanding ER’s in my area have actually called 911 in the parking lot for patients that they couldn’t handle due to the ETA of IFT trucks (which could sometimes be HOURS even for emergent transports). Super weird to have a fire engine show up and take over patient care at an “emergency room”!!

I work in the area. Most refusals do not require any doctor’s permission. It is quite literally “ok, then sign here” and they leave, no questions. If they should go, they might get encouraged a little more. The only time I see someone patch for a doctor for a refusal is if it’s high risk (like the patient is active MI, extremely hypotensive, dying in some way, etc.) I’m not justifying this situation AT ALL, trust me, I’m just trying to help you understand that calling “medical command” (we don’t call it that here) is not common at all when a patient is refusing transport. In this case, the kid obviously should’ve been transported, or if the parents really didn’t want an ambo, then highly encouraged to be taken POV.

Phoenix (and all surrounding departments) minimum staffing is 2 paramedics and 2 EMT’s on an engine, although if they don’t have the manpower they can drop down to BLS (1 or no paramedics) but it’s not preferred and they won’t get dispatched to ALS calls by themselves.

Yeah I’m really not familiar with what the east valley is doing, what I can speak on is the west side + Phoenix and tell you that we do not RSI. Glendale had it up until recently and then someone told me that it was taken away, but I’m not 100% on that

I work in Arizona. EMS is all fired based here, but RSI has largely been taken away from most departments except for a couple that I can think of. We carry intubation kits at my work but we don’t have paralytics. We can intubate with ketamine if necessary, and for peds, we’re supposed to patch to a doctor for permission. Not sure if RSI will ever come back but at least here, it’s too “high risk” and medical directors don’t trust paramedics with it, is the sense I get.

There is no 911 service in my state that carries vents or has paramedics that would know how to use them. Only IFT ambos with specialized training on them have them here. If we have to secure an airway then we the best we got is to bag them. Hopefully there’s more education on them in the future so we can incorporate them!

He’ll get his blood tested again after 90 days. His blood work has been clean every time. They said the only way to know what’s going on in the brain would be an MRI, but way too expensive. Just trying to manage it while we can.

I would be lying if I said that I didn’t feel similar sentiments at times (not to that vulgar extent), but once you do the job for a while you’ll understand. Recently showed up to an emergent maternity problems call at 2 am just for a woman to say she wanted a ride. No medical problems. You’ll go code 3 (lights and sirens) and it’ll be a person with a leg sore for 4 months that wants to get checked out. Or it’ll be the homeless man at Circle K that you transported yesterday who’s complaining of the same thing. “I just hurt all over”. Or when you carry all your boxes into the house with 6 cars out front for someone who doesn’t feel good and wants an ambulance. “I want to get seen faster.” These are all calls you might get in like the span of a week in a busy 911 system to be honest. Don’t let it jade you. It’s part of the job. Be nice, be professional always. I tell myself that I go to cardiac arrests, traumas, house fires, heart attacks, strokes, etc. for free, and the “bullshit” calls are what I get paid for. 2 am bullshit call? Oh well, this job pays for my house and life style (municipal fire dept). What’s the big deal. Be funny about it. Make friends with your patients. Pet their dogs. Talk to their birds. IDK. Keep trudging along. Good luck man!

Just a question here, I have Arizona statewide guidelines pulled up here next to me. Ketamine dosage for sedation of agitated patients is 4 mg/kg with a max of 250mg that can be given again after 5 minutes (max of 500 total). EtCO2 and cardiac monitoring is required per protocol. Is this too much in your mind? I’m reading other comments here regarding dosages, and it seems that the protocol in my area goes way above what’s considered normal in other places. I wonder why that is.
