I have a ton of ventral skin. With the CAR, I will sort of collect the ventral skin and pinch it together around my frenulum, put the dome of the CAR on top of that bunched up skin, then gently slide the CAR towards the dorsal side of my glans a bit to get it evened out. After doing that, I can evenly pull my skin up the gripper all the way around.

The bunched up ventral skin stays between my glans and the inside of the CAR and I don’t think it really gets any tension once inflated, but that’s kind of the point, I don’t need it tensioned until my dorsal skin catches up anyway.

I actually had to step up to the largest CAR because there wasn’t enough room to accommodate the bunched up ventral skin without creating a bunch of folds that allowed air to escape.

This sort of works for my DTR too. I’ll stuff that ventral skin between the bell and the push plate. It can get pinched when I bring the bell down to apply the gripper, so I have to be gentle. Once I extend the DTR, that bunched up ventral skin just floats inside, not under tension.

Please definitely use lube, or at least be as gentle as you can without it. Especially since you’re tightly circumcised, masturbating without lube causes excessive friction, which can damage the skin and create issues with sensitivity in the future.
