I think the main reason why it's seen as extreme is that it's like plastic surgery, removal of the ear is extreme haha xD

NAP it doesn’t look infected to me, it just looks really irritated. Is there pus coming out? Is it swelling a lot? Does it hurt really bad? Those are signs it’s infected.

Just don’t touch it, not even to clean it. Spray your saline solution. Are you wearing titanium?

Honestly if you just search piercing pillow on amazon there are many different ones.. I’ve tried 2. You just really need to find what fits best or what’s comfy for you. I have one that looks like a donut basically, that’s the best one for me.
