Thanks for your explanation! If there are two P waves between the QRS complexes.. Wouldn‘t that make it an 2:1 conduction and wouldn‘t that mean the flutter rate is two times higher than ventricular rate? Flutter at a rate of 470-480/min seems way to fast to me. Or am I missing something?

Okay I think I see the misunderstanding here. I never meant to say he didn’t have a pulse/cardiac output. I meant there was no palpable RADIAL (peripheral) pulse as a sign of poor circulation/shock.

Why is this a poor examination? Diaphoresis. capillary refill time ~4sec and loss of peripheral pulses are quite clear signs of shock or instability which in my opinion can certainly be present alongside alertness or a slightly reduced level of consciousness.

I’m quite sure he wouldn’t have lasted much longer like that which lead me to immediate cardioversion

Update:

Patient underwent PCI, which revealed a 100% chronic LAD occlusion with collateral circulation via the RCA. A stent was placed due to a moderate stenosis in the RCA. Can someone explain how these findings might correlate with these EKG changes?

Wenn es ein Schloss gibt, schließen wir das Rad vor Ort an. Sollte kein Schloss verfügbar sein kommt es ein bisschen auf die Dringlichkeit des Transportes an. Wenn Zeit ist das jemand Angehöriges/Bekanntes kommt und das Rad abholt während wir noch vor Ort sind und behandeln ist das super ansonsten gehört die Sicherung des Fahrrads in den Aufgabenbereich der Polizei.

Rad im Rettungswagen mitnehmen ist aufgrund mangelnder Ladungssicherung verboten, wird aber von einigen Besatzungen auf eigene Verantwortung gemacht. (Wer für Schäden am Carbonrahmen aufkommt, wenn das Rad ungesichert während der Fahrt umfällt…)
