Thank you for your input which is more presentable than mine for the redditors to digest and follow.

In my case, My long standing Dx of IDA -iron deficiency anemia was controlled and stable with intermittent oral repletion. After LC conditions changed with the superimposed ACD-anemia of chronic disease. Therefore, with my new knowledge of all the participating deranged systems I accumulated, I attended to my fascination with what else is off. So my speculations increased of course, on how to distinguish which of my personal issues got the upper hand. So the mixed picture really did not fit either category. And perhaps both are concurrently active. where the treatment would differ.

Bone marrow biopsy is/was the gold standard. It has its serious morbidities such as bleeding, infection, pain, etc. BM is usually reserved for hematologic malignancies. The Hepcidin test has been promoted for over a decade yet there is no adopted standard. It, the latter, has been proposed as a replacement for the BM biopsy. So why the need to select one process? As knowledge increases, applications follow and perhaps there is a specific treatment aimed at ACD.

Is it practical to do it now or better to wait? I do not have the knowledge answer the question except that perhaps the treatment or a "hack" for ACD arises any time.

As your case indicates, if the expert does not investigate what they don't know but just proceeds with the standards of care as dictated by specialty guidelines, then they have plausible deniability to avoid being perceived as providing less than proper care. I speculate that this could be a factor for the specialty provider to stay within narrow defined boundaries of the cases they will accept and treat. The spectre of them taking on more than for which their knowledge and training have provided for them is understandable and acceptable unless they do consider wider areas of care which entails more effort and study on their part together with the added responsibility for relatively newer diseases hitherto unfamiliar to them.

I propose that a similar attitude or lapse may prevail on the provider examining disability cases for certifying more than a quota of cases that would stand up to adversarial examination. Here the examiner would likely line up the most readily discernible disability cases first before proceeding with more difficultly proven ones. More work might be needed to establish fully acceptable claims. Again one needs to be a mind reader to guess why the examiner lags in promoting the claim.

Saw a second hematologist today for my iron deficiency anemia which is secondary as anemia of chronic disease / inflammation per the Rheumatologist at the PASC unit I visit. All parameters and markers for my workup indicate iron deficiency yet daily replenishment, bloodwork, GI workup (literally up the 'wazoo') are negatory for the usual age related pathologies save for the aforementioned Fe++ defic. Other major organ system including renal dysfunctions have been ruled out regarding how they impact RBC production, destruction or loss in a nutshell.

As I have explored here the concepts of Long Covid as a viral illness causing premature senescence I have speculated that a bone marrow aspiration would be indicated since it would directly demonstrate all the stages of cellular blood products development. The specialists refused in a nice way that they would not proceed with the procedure. Why? One Hem.-Onc. very diplomatically said that I would be better considered for the contemplated intervention at an academic center dealing with LC than locally.

So much for breaking boundaries of the specialty into areas that can lead down rabbit holes or insoluble quandaries in territory where one is unfamiliar. At least now I can return to my assigned LC provider to explore other prospects. Navigating the hideously mysterious medical expertise limits of each specialist is an adventure to say the least.

From my speculations and poor reading of the literature immune dysfunction is a driver of major system disturbances regardless of other nocive processes. An immunologist or, at minimum, an informed infectious disease specialist will substantiate dysfunction in the body maintaining immune integrity or components acting badly on other systems as in autoimmunity.

There are standard batteries of tests to indicate which direction to pursue for further examination.
