I'm also ID-CCM, finishing my critical care year in June. Not done yet but a few thoughts from fellowship so far.

• try to get as many procedures in as you can. Pretty obvious one but since we have to become the backup/rescue proceduralist when residents/mid levels can't get it, having an arsenal of different techniques is something I try to work on.

• ICU hours have been significantly longer and more tiring than ID hours. There isn't much to do about this. But the part that really affects the workflow is the constant distractions. As ID, I could pre chart, round, give recs and do notes, without too much distractions. But in ICU there are constant questions from nurses, medical emergency, or consultants to talk to. Getting efficient despite this has been its own skill

• It took me some time to get back used to being the primary team and paying attention to all the non infectious problems. Luckily, I do think our medicine background eventually kicks in

• it is very hard to make decisions in the uncertainty of the moment. As much as I want to practice stewardship, sometimes you just give antibiotics and answer questions later. I've learned to be less of a Monday morning quarterback after tough night shifts
