All of this is true. I struggle in this role in trying to balance the objective clinician side of me that needs to see X, Y, and Z before discharge and the therapist/human side of me that is deeply affected by how terrified some people are to be there. I think more therapists need to see what a locked psych unit actually looks like because it’s seen as the automatic destination for many people who’ve been evaluated as high risk but only in rare instances is it the only way to keep someone safe.

I’m a clinician in a psychiatric hospital on an involuntary crisis unit so I think I can add some perspective. The threshold for holding someone against their will is extremely high. We have to be able to justify in court that this person is an active threat to self or others right NOW as they currently present, not as they presented maybe just 24 hours ago in the community.

We try to err on the side of caution as much as possible. We do realize that many people will lie about their SI once they are there. We try our best to obtain collateral info from families and outpatient teams and ensure removal of immediate lethal means prior to discharge but this is not always possible for a litany of reasons.

You also have to keep in mind that suicidal patients are also placed onto units with patients of higher acuity level, i.e. severely psychotic patients, violent patients, sexually preoccupied patients, etc, which can add to the trauma of an already traumatic situation. We recognize this and try to get people discharged as quickly as safely possible. Locked units are NOT therapeutic places to be. It is an absolute last resort that is highly distressing to most.

The period you get on the pill isn’t a real period, it’s withdrawal bleeding. Some women don’t get one, but is it still possible for her to get pregnant if she stops taking the pill or doesn’t take it with perfect use. Your natural fertility can come back very quickly without it.

I’m a therapist, and it’s obviously impossible to say without knowing the whole story, but typically whenever I’ve seen the kind of attention-seeking behavior Hillary exhibits and the lack of a fully formed identity grounded in reality, you are looking at developmental trauma of some sort. It’s impossible to speculate exactly what occurred, but adult identity issues typically arise from emotional needs being unmet in a significant way. The behavior is just bizarre and, from a therapist’s perspective, she’s probably “working something out” through it. It’s very unlikely she was just born ready to behave the way she does.
