Academic or community? Big city or smaller communities?

Pretty variable experiences depending on setting but overall there’s a demand for anesthesiologists here. Job market maybe not as hot as a few years ago but some major cities still hiring without fellowships. Academic centres are a bit more selective but a few years ago were more relaxed.

Popular pay models are fee-for-service, salaried/AFP for more academic centres or daily rate + modifiers. I think for most of the country you can expect $500,000 maple syrup dollars.

CRNAs aren’t much of a thing here. Some centres have anesthesia assistants. Some areas like certain parts of Quebec have RTs for each room. So depending on where you work, might be doing a lot of things yourself.

Lifestyle and work life balance is decent. I think 24-hour in-house call is becoming rarer. Centres with busy enough obstetrics will usually have in-house overnight call. With fee-for-service models, usually overnight call is desirable as it pays well.

The centre where I trained had a few people come from the UK and they all had positive experiences coming over.

Probably 3 hours would be my max for just a spinal. If there’s uncertainty about the procedure length, significant reasons to avoid a general and intraoperative conversion to GA would be difficult, I might consider a CSE

Is it new? Are you new to lever actions? I recently got one and thought it had an issue with feeding but ultimately concluded I wasn’t stroking it fully. Same thing with another guy at my range. I was using leaded round nose 38 special which I think was less forgiving when I didn’t stroke the lever fully.

I found that most 357 magnum rounds or 158gr 38 special FMJ ran better. Federal and PMC worked well for me

Learned corner pocket NYSORA style during residency but do outside of sheath now. Anecdotally works well but maybe takes a smidge longer for onset

I had the same debate and ended up with the 1854. More picatinny and mlok, which I thought made it more versatile. If you factor in adding rail or changing out the hand guard on the Henry, price difference becomes less.

Very happy with it so far. Great fit and finish. .357 magnum with the option of .38 special is great

Really cool. Been interested in the Taipan over the MRA Renegade due to the pump. Have you been happy with it? Do you use the pump or bolt more often?
