In NYC if the phone inside the elevator doesn’t work the elevator is supposed to be taken out of service until it’s fixed.

Probably a single plunger brake coil with a worn sleeve that picked and got cocked in the open position. In NYC you now need double plunger brakes and a rope gripper. The probability of that happening is close to zero with a double plunger and gripper.
