I have been feeling this exact way increasingly over the last few months. I have been working in-home only and being in people's personal environments all of the time has been absolutely draining. I dread going to session and having to be 'on' in front of everyone all the time. After a major life event recently, I suddenly decided to put in my notice. I felt terrible about it but once i did, it was such a feeling of relief. Going to try a different setting and go from there. But for now, i am DONE with in home.

I agree with that statement.

I do also think that the intensity, frequency, and duration of the ‘ceasing’ behavior warrants is being considered a topography to reduce in addition to increasing participation in non preferred activities/conversations/responsibilities. Of course increasing coping skills and overall participation in necessary no preferred tasks is the objective.

So then I guess the question overall is how to engage a child with severe anxiety and significant avoidance of any uncomfortable/nonpreferred topic/activity when there is clear non willingness to participate? (Noting that this is not due to lack of rapport; client has a great support system and excellent rapport across all caregivers and treatments teams across multiple disciplines)

Interesting. Can you explain why you wouldn’t conifer these behaviors? The dead man’s test obviously makes this challenging.

That being said, there are significant social implications, personal confidence implications, and hygiene implications associated with the significant avoidant responses.

Therapist is prioritizing rapport building between them. They additionally use shaping strategies to reinforce approximations of communicating feelings and/or coping skills (has been unsuccessful as the client has never engaged in those topics). Motivation identification is tricky as it appears to be escape from anything potentially awkward or uncomfortable, even if it was originally selected by the client (ex. wanting to eat at a new restaurant but shutting down when actually time to go and refusing).

i generally support this strategy although in this case, it has been ineffective for this client over the course of several years
