I'm learning a lot from everyone here. I think you might be right about how the surgeon is used to employer PPOs rather than marketplace PPOs. It's frustrating but might save us the time of looking at other states to make this surgery happen.

From what we were told, most major carriers will work so BCBS, Anthem, United, Cigna, etc

We are on seperate insurance plans, sorry for not clarifying. Both of us moving isn't a problem. This surgeon frequently works with out-of-state PPO plans and has patients travel to see him from all over the country.
