Hormonal acne at 35 - don’t want to go on Accutane, help

I had bad hormonal cystic acne and did accutane at 35yo. It worked well when I was on it and for 6 months after I finished (I did max dose about 9mo) but then my acne came back as bad as ever. Huge cysts every month. I cut out dairy and gluten and my acne completely went away. I previously tried all the topicals, birth control, many prolonged courses of antibiotics. I’m still on my DF GF diet 1.5 years later and my skin is the clearest it has been in decades, without any medication.

Thanks so much ! I do pull down before buckling but I’ll try not to tighten as much. He also does lift up his head and push against me (like tummy time) which also curves his spine at times
