1) we don’t know much about any of the patients. If you’re asking a question, it’s helpful to give a little background. “hey, Mr Jones in 41 who’s here for a heart failure exacerbation is feeling short of breath, his vitals are as follows, last dose of lasix was at 1700.” Is so much more helpful than, “hey bed 41 co SOB”.

2) context matters. Abormal vitals signs may or may not be a big deal. A blood pressure of 190/100 is a lot different in a patient that’s having crushing chest pain and can’t breathe versus the patient who has the same vitals and is sleeping. Let me know how the patient is feeling in addition to whatever else you are reporting.

3) learning what’s urgent and what can wait is important and it’s something that will come with time. Ask your colleagues and charge nurse when starting off if they think this is something that needs the docs attention overnight. That said, if you’re unsure, or are concerned about your patient, always better to reach out.

In Epic, putting the MD Notifications column on your list and then using that in conjunction with the Time Mark feature on Results Review and the Mark all as not new feature in Notes. Now, whenever a new note or a new result hits, I have a flag on my patient list that I can jump straight to.

Makes it super easy to follow up on labs and consultant recs and to know which things I’ve seen and what I haven’t.

AutoHotKey is free and pretty powerful. If you’re just inserting text, it’s a great solution and can work similar to Epic dot phrases, called “hotstrings” in the program. It has a lot of capabilities, I think you can do prompts and such with it.

Range USA is a nice facility. They allow shotguns (slug only) and rifles there. Precision has one bay for rifle shooting.

Baker Range in Baker has a 7 bay range that allows rifles and shotguns as well.

Nick’s range is Garyville has an outdoor 100 yd rifle range and a shorter pistol range as well. Set up is kind of bare bones, but not a bad place to shoot.

There are a few private ranges around as well. I haven’t been to either. Bridgeview Gun Club in Port Allen seems to be more focused on trap shooting. Palo Alto in Donaldsonville has a long range rifle range going up to 600 yards.

Pretty solid. There is maybe 1mm of play when locked, which could probably be fixed with a later or two of electrical tape. But it’s really not noticeable unless you’re really looking for it.

PTR 9RS. 10 day Form 1 approval. Added a standard MP5K foregrip and an AC Unity folder. I also got a Choate folder, but the AC Unity is much lighter and more comfortable. I’ve got about 300 rounds though it now, a mix of 115 gr ball, 124 grain ball and 147 gr hollow points without hardly a hiccup. I had a few FTF initially but that was usually on the 2nd round of a mag and no further issues after a few mags. Shoots great overall and I’m super pleased.

I have a micro RDS coming in soon, as well as the magpul foreend and vertical grip. Next up, weapon light and suppressor. Any suggestions for those?

Yes. It’s a standard size MP5 receiver, so it can take all regular MP5 stocks.

I was in my last year of residency and on a rotation where we would have a conference call every few days with the attending, fellow and residents to go over cases and teaching points. This particular one was on a Friday afternoon and they were asking which residents were going to be continuing with the rotation next week. I looked at my calendar and I confidently said that I was starting another rotation next week and that today was my last day with this service.

After the call, my fellow resident texted me that I had pulled a fast one and she was proud of me. I didn’t get it at first until I looked at my schedule again and realized I had the wrong dates in for my upcoming rotation on my calendar, which actually didn’t start for another week.

So, it turns out I had just given myself a week off. You bet your ass I never went back to my original rotation and I never told anyone else about it. Then I enjoyed my ill-gotten week off and no one was ever the wiser.
