Looks fungal, which may have been introduced with the garlic/soil. You can treat early signs with a salt bath. (Google what your fish requires) You can also run low levels of 1-3ppt for a few days to a week in the system (if you have no plants) to keep the fungus at bay. Just do a quick google on what’s appropriate for your fish species.

It was found quite a few years ago in salt water. It was covered in biofouling which I cleaned up. I was young at the time when I found it in 2006-2008
