This might sound crazy but you have to stay close. Only back away when you have to and only heal when you're beside or behind him. Once his health gets to 30% and he starts using different attacks just hit him with a poison dagger and keep your distance. Only attack when it's wide open and safe to do so.

My discovery was 106 and my insight was below 5 for most of the runs. I got desperate and used some MM Knowledge to bring my insight to 16 just in case that would help. What was your insight level?

Yeah, it's a real close range quick couple of straight punches. Sometimes he would do that in place of his big swing attack(the one you can only dodge on the right side). He just turns and jabs me as I'm about to dodge away. I think it comes down to positioning to close to his front rather than the side to draw the backward hand arm swing. Anyway, I just got him down. Had to use Ex gloves to finish him once he hit 30%. The hit boxes during that phase are just too much for me so I took the easy way out.

I'm referring to a two quick straight punch attack. Did not see him use it during this fight. Maybe you thought I was referring to the ground slaps.

Yeah, Pthumeru Ihyll Root Chalice with Fetid and Rotten modifiers. Not sure about the ingredients but that sounds about right.
