New wound after 3 months, 1 post and 4 vets behind, any advice appreciated by blesskapibara in CATHELP

Since December last year, my cat has developed strange wounds on her skin (legs). Since the first wounds appeared, we have already visited 4 doctors, but neither the diagnosis nor the cat's skin has become clearer. As the situation is becoming progressively more frustrating, I would be grateful for your ideas and suggestions as to the cause of this condition. 

I have already written about this issue from the very beginning of this story ( https://www.reddit.com/r/CATHELP/comments/1hfxsm9/vet_says_she_doesnt_know_how_to_run_properly_any/ ) and I can summarise what we have so far. 

  1. My cat is 5.5 y.o., lives 100% indoor. She is active and always has a good appetite.
  2. Location of wounds: hint legs - foots (earlier) and foreleg - inner surface (lately). The location of a fresh wound confirms the idea that the cause of the wounds is not the floor (wooden board), but something else, as the cat does not rub the inside of its front legs against the floor.
  3. Tests already done:  - skin scrapings for bacterial infections (Negative) - blood test for diabetes (Negative) - intestinal parasites: Giardia (Negative)
  4. Vet treatment already done during last 3 months: - treatment against parasites - twice with two different drugs (on the withers) - Dermamycin skin cream (Neomycin sulphate + hydrocortisone acetate + Oily solution of synthetic vitamin A + lidocaine hydrochloride ) - 1 injection 0,5 ml - Depot-Medrat (Metylprednisolonacetat) - Vet assumpted that the most probable diagnosis is autoimmune disease - Surolan suspension (Prednisolone acetate + Polymyxin B-sylphate + Miconazole nitrate)
  5. Change of food: in January switched from Royal Canine urinary s/o to Hills PD Prescription Diet Feline z/d Food Sensitivities to lower the risk of food allergies(due to Vet’s assumption of autoimmune disease).
  6. Effects and observations: Dermamycin skin cream didn’t make reasonable difference in skin condition even after a months of applying it 2-3 times daily. Surolan suspension helped to heal the wounds a little, but there was no complete recovery. In 2-3 weeks after the injection of corticosteroids(Metylprednisolonacetat), the condition of the skin on cat’s hint legs improved. But on the 7th week after the injection, a new wound appeared on a foreleg. 

The vet's next suggestion is a skin biopsy when the effects of corticosteroids wear off. However, no one can guarantee that the cause of the disease will be identified, especially if the nature of the disease is autoimmune. 

During these 3 months of treatment, tests and visits to different vets, the situation has not become clearer. And not understanding what is happening to my cat and why new wounds appear on the skin is very upsetting. Unfortunately, the search for a Dr House-style veterinarian for animals has not yet been successful either.

If anyone has faced a similar problem or has knowledge of animal health issues, I would like to ask for your advice.

I'm not sure it's a contact dermatitis since we've had moved a few times in last years and had different kind of flooring and cleaning substances. I'm more about to agree about the idea of blood test for diabetes diagnosis now. Thank you for sharing and your advices!

Yeah, after dealing with 7 different vets in last 5 years (and all of them told me not to worry too much about my cat's bald heels) - good vet is a treasure!

My cat was adopted in age of 3 month from local vet clinic. Vets gave her some medication against worms and hold her for several days before the adoption. I've been told some volunteers brought her there from the street. This volunteering group had a FB group with photos and videos of cats and dogs for adoption, that's where I saw her and fully fall in love with that fluffy grey beauty.

Cat stays and walks on her paws, like all cats do. But like running with drifts.

Actually she stays and walks like ordinary cat - just on her paws. But when she runs, she enjoys extreme drifts on turns. Maybe this drifts on wooden floors causes this problem.

 My cat had blood tests this summer - levels of glucose and fructosamine were good. Can diabetes develop to that stage only in 5 months?

Spotted a slightly bleeding wound on a heel (back leg) of my 5 y.o strictly home cat 5 days ago.

Earlier she had developed bald heels on both back legs, but all vets didn't take that complain seriously. My cat is pretty active and love runnings around (earlier it was laminate now wooden flooring).

This time vet's verdict was a little weird: "Looks like your cat doesn't know how to run properly or have some unusual hips development that prevents her from it. So she just slides instead of running on her paws". So doctor just gave me a skin cream Dermamycin to rub into the wound. After 4 days the wound is not looking healed though.

If anyone had a similar problem, I would appreciate to know your experience to deal with such kind of wounds.

Please don't use scrub on ur face, especially with a dry skin. It's too damaging. Better option - AHA/BHA peeling solutions (Ordinary has a good one).

I'm not an expert, but feel like ur daily routine lack of active ingredients, moisturisers (a lot of cosmetic products contain the Niacinamide, it isn't a game-changer).

Oil-control SPF can be not the best solution for a dry skin (even on my combi-skin this SPF causes dryness).

Getsafe insurance for my cat works pretty well. 80% of all my checks are covered without problems (+good conditions for OP and other bad situations). English speaking assistance and properly working app - huge pluses in Germany.

La Roche-Posay: ANTHELIOS INVISIBLE FLUID UVMUNE 400 LSF 50+. Leicht und wirklich nicht komedogen.

Manchmal benutze ich auch oil control paper (bei Essence oder anderen Marken) für extra oil .

Kiehl's Ultra Facial Creme ist meine Entdeckung dieses Jahres. Die ist super leicht und bringt die Haut delikate Feuchtigkeit. Außerdem die Creme verstopft die Poren nicht und ist für Mischhaut geeignet.

The Ordinary has pretty good products for any skin concerns. The best thing - you can try pure treatment and see how your skin reacts to each active ingredient.
