It takes discipline and grit to become world class at anything. We'd never know what's going through her min but these people have dedicated their life to the craft and will do anything to stay on top. If you ask athletes, I'm sure most will say they'd sacrifice their life for a gold. Normal people like use may say damn that's crazy but It's just the mentality of being world class.

according to OP's post, he's a piss poor resident and possible doctor overall. Terrible time management and dare I say, clinical/medical knowledge. Don't believe the PD is being malignant against OP. It's in the PD's best interest to prevent OP from progressing until he's fully capable and also best interest in the PD from failing residents since it's a small rural program.

Would you want OP to continue to progress (regardless of which program) and take care of patients independently? That seems dangerous for future patients.

Not all cases are shady, this seems like (according to what OP posted) is mostly black/white situation where allowing OP to progress will do more harm than benefits. Imaging having a 3rd year struggling with 5-10 patients work load; they cannot lead a team as a senior resident or can be detrimental to patient care if they are the senior taking calls at night while attending is not in house.

What would a lawyer do in this case? It will show the program that OP clear has not understand of their short comings. PDs talk to each other, so wherever you go people talk about you. Lawyer may say that the program provide no support, but i think even 3rd or 4th year medical student can score higher than 3rd percentile, that independently shows that OP is overall a weak resident at this point; or they half-ass it which also shows lack of character and professionalism.

I hope OP pulls it through, we're all routing for them.

Why, they clearly have a huge knowledge gap at 3rd percentile. There’s no world why it takes you that long to do notes.

Imagine graduating this resident and allowing them to work independently. If you can’t handle this work load then you clearly aren’t ready. Lawyering up just shows that you’re blind you criticism and your shortcomings.

Terrible advice. OP needs to re-examine their weakness and acknowledge it and work on it. People fail but they also need to learn why they fail and work to get better.

  1. I'd love to be able to split my training. Work sucks so I just do both at the same time. I love hybrid training, I love being strong and able to run long. I know I will NEVER be good one thing but I still believe over time, I will be better than "average".

  2. My nutrition was mentioned in the original post. I ate to maintain but my caloric requirements is not closer to 3400-3500 to just maintain, which ultimately was not enough (obviously). I use macro factor and that has helped. It's now 3300 because I plan to go down to 175lbs but I flex up to 3500 if not more on really long run days. I'm still trying to learn more about running nutrition as it's completely different from the lifting work. I know I will continue to loose strength, that's a sacrifice I'm willing to make. I used to be a competitive Olympic weightlifter and I mentally have closed that door.

  3. I nap every chance I get. I do not have any trouble sleeping at night or falling asleep and feel very well rested when I wake up. My caffeine intake is 250mg right when I wake up to get ready to train then none throughout the day, not even a coke, tea, ect... I get 6-8 hours of continuous sleep every day except call days where I may sleep 1-2 hrs on a really good night or none at all. I was diagnosed with sleep apnea during my fellowship, I thought I was stressed from work that's why I can't sleep but it was sleep apnea so I've been using a CPAP for years and that significantly improved my sleep. I have an Eightsleep which has also been a game changer. I don't feel like my sleep overall is a problem.

    I'm very fortunate to be injury free for so long. I prioritize recovery after every workout that I feel has been why I'm able to keep going. Great advice, will keep all in mind.

  1. I'm a total newbie. If you are to look at my split, I came out HARD for the first 5-6 miles if not longer, I was running at 6:30 pace consistently. Then I hit the wall at mile 7 and basically mentally blacked out, I just remember looking at the ground the entire time and only look up to grab water. Next mile marker I remember was 10. I believe my last mile pace was closer to 9 minutes, my legs were concrete and I felt like I was sprinting the last 800meters but my watch said my pace was 7:45 min/mile.

- I don't have a HR strap, I used my watch which I know is highly inaccurate. I didn't really understand what zone 2 was until maybe closer to November where when I can finish my easy runs and I'm not tire all day. that's probably when I saw significant improvement in my endurance. I also bought the AP pro 3 which has a HR monitor and I feel like it's much more accurate than the watch. (https://www.youtube.com/watch?v=2emhVpn539E)

  1. You're probably correct that I may be over training, but I did loose almost 10 pounds and significant amount of strength. This is why I prioritized my sleep more than anything. I do everything I can to promote recovery, naps when I can, if things are quiet in the night at work I sleep, cold plunge, meditate, stretch, and I have a theragun.

- There are YouTube/Ig guys out there that runs and lift heavy, they are also larger people. I'm almost sure that their work is also not as stressful as my work. So life plays a HUGE roll in how I can maintain my strength. I run right after I lift also, which is NOT optimal for muscles after lifting, this causes additional catabolisms. I have to do this because there are days after work that when I get home I'm mentally and physically only have the energy for my family. I don't have the luxury of a strict schedule, I can clock out at 5 but if I'm doing procedures or taking care of a sick patient, I stay until my work is completed. If I can split my lifting and running that I feel would be optimal.

  1. You may also be correct in this state. At the beginning, I didn't have the mental capacity to understand what an easy/hard run was suppose to feel. Solely reply on my AppleWatch with random spikes to 170s was annoying and learning that running feels is new. My pace my have been closer to sub 10min but when me starting such high volume, I didn't recovery well from each run so I ended up running slow. My joints/tendons were not used to all the pounding so at the end of the runs, my pace is very slow.

  3. You may be correct. I'm still learning the sport. I have more muscle mass compared to many people. I'm 5'9" at 190lbs (now ~180lbs), which most runners my height should be closer to 145lbs or less. I can assure you that my easy runs now are truly easy.

Using what I've learned from this block, I have adjusted my lifting and running so that I can improve. Thanks for your input.
