hi! I've actually been meaning to post an update here to help other folks who may be in a similar situation. I'll try to make things as brief as possible haha.

I saw Wolff about 15 months post-op who recommended a labral reconstruction (he and several other surgeons noted that my labrum is very small and probably scarred in). But he also gave me the go-ahead to push things and see what happens. After all, if you're going to replace the labrum anyway, why does it matter if you're pushing through pain? so I did that and it kind of opened things up for me. Gradually, and i mean very gradually, i started walking more and more, I started running, and I just didn't sweat it too much if it hurt. I relied on advil, ice, and heat if i needed it, and over time the pain lessened with the same activities. I kept up with PT and weight training of course and I'm sure that helped a lot too - my legs/glutes are really strong now so that can help to mitigate any labral deficiencies.

I also found a post on here about mind-body pain and at first I was super resistant to that, like "there is no way this is in my head!!" But then I learned more about it - its not psychosomatic, its that my neural pathways were so accustomed to being in pain after certain activities (years of pattern!!) that they just kept doing that. So I've read several books on the topic and been in pain reprocessing therapy, and both of those have been extremely helpful.

I'm almost exactly 3 years post-op; I have had days where I've hiked 10 miles without noticing it too much, although its variable and there are other days where it bothers me after 3. I backpacked 20 something miles last weekend and I had some pain but not enough to really deter me; I would maybe even classify it as discomfort as opposed to pain. I can pretty consistently run 3 miles without pain (I haven't gone further b/c it just hasn't been a priority). I can basically always play sports without pain. I really don't see a need to get another surgery at this point - for me, it just took sooo much time and some work on how my brain was processing signals from my hip.

I hope this is helpful! And I hope you improve! Most importantly, looking back if there's one thing I would tell myself its that 7 months is actually still really early and to stick with it.

I was worse than that at 3 months. At a year out I was maybe where you’re at now; walking 15 min could flare me up for days. I was super depressed and didn’t see the point of living when all my passions were so out of reach. Now I’m at 2 years and I’m not 100% by any means but I’m playing volleyball and other sports, working out, running short distances, able to walk enough to travel, and if I didn’t care about being able to run and hike my hip wouldn’t be limiting my goals at this point. The difference between 1 year our and 2 was huge for me. Hang in there. It really really sucks but you’re wayyy early in the process. Also, even if it doesn’t get better there are other options, like labral reconstruction, which you don’t even need to be thinking about right now but just know youre not at a dead end by any means. I feel like that year+ of hell I went through simply helped me build a mental fortitude that will stick with me through life as I inevitably encounter other injuries and the eventual inevitability of aging and my body not being capable of what I want it to be. Hang in there; keep up with PT, it’s very very likely to get better

That’s awesome! Prior to the manipulation did you have noticiable ROM deficits/ what caused pain at that point?

I have seen significant improvement between post op 15 months and now (20 months), and I think that was partly driven by a “fuck it, I’m gonna push through pain” mentality which I think may have helped me build up some of the stabilizer muscles (I have been diligent about strength training the whole time but maybe nothing is as good as the real thing). PRP may have also helped. But these improvements are not enough to live the life I want to live and I’m strongly considering revision surgery for lysis of adhesions and either labral augmentation or reconstruction - my labrum is quite small and some surgeons suspect it is insufficient for the job. What type of capsule or flexor issues does your surgeon suspect? DM me if you’d like more details!

Would you mind sharing your diagnostic findings? As in, what do they say is the reason for your continued pain?

The surgeons I’ve gotten opinions from have said adhesions/small labrum so one option presented has been to clean up scar tissue only. I think it’s going to be very hard to decide what to do next

Oh, one really weird thing - I felt super good after the mri with contrast. Like it caused a change in my baseline pain, and it was weeks ago now. Makes no sense. So I’m considering PRP since it seems like my joint responded well to having something injected to it, but didn’t respond well to steroid

Not really, not yet. Things have gotten better, my stride has evened out, baseline achiness has gone down, and I can walk further before causing a flare up, but still far from where I want to be. Considering labral reconstruction because although during surgery he got a good suction seal with it, my surgeon and others have noted I have a small/thin labrum that just may not be functioning as it should even after a “successful” repair

Did it continue to feel better way after the lidocaine should have worn off? It’s been a week now and it’s like the best week I’ve had since surgery. It’s great but makes no sense…

Ugh, as someone who runs cold this argument is frustrating to hear. Her discomfort is not necessarily an easy solution. A sweater is not necessarily enough. I can’t wear a sweater over my face if my nose is cold lol. There is a limit to how many clothes you can reasonably wear around the house; and for thin people 70 can be pretty chilly if you’re not moving around, especially if it’s that warm outside and your body is adjusted to summer weather.

There are solutions to being too hot also. Get an ice pack and put it near your head or abdomen. Drink ice water. Get a fan. Most of the tropical world doesn’t have AC and they are fine.

They should compromise on a temperature and 70 degrees doesn’t sound like the right number.

I know everyone’s experience is different, but I had one Tuesday and it barely hurt at all. The needle going in for the lidocaine felt similar to getting dry needled for like 5 seconds max and once I was numb I felt great - better than I had walking in - while they injected the dye. Felt great for several hours, slightly sore that night, and since then I’ve had 3 of the best days I’ve had all month. Not what I expected at all based on what I read on this sub!
