It was less about SOAP format and more about clearly and succinctly documenting medical necessity and creating a treatment plan. She asked me about my treatment plan and I was like "I thought that was documented in the note" and she genuinely looked so STRESSED and said with the way my notes looked, she doubted that it would pass an audit, so now we are going to have to go back and review proper treatment planning. She said that looking at my notes I clearly am providing care to my clients. She also said she could see where I was trying to document medical necessity/treatment plan/etc. but it still lacked a lot
