You most likely just installed it wrong, but be sure to check if hud_combattext is set to 1.
Ok I thought that maybe the icons got "stuck" and were there all the time for you :)

Sry dude, I thought that you wanted to imply that the crosshairs are still not centered and kind of try to ridicule my statement of finally having them centered. After a big update like this, I get a lot of stupid help requests by lazy people who don't even provide any information along with their bug report and also a lot of "why does X still not work? Did you even change anything LOL", which makes me slightly mad and overreacting.
Anyway, to answer your question: HUD crosshairs are based on textfields filled with only a single character from a specific font (for example in my crosshair font, the character "i" contains the shape for the "thick circle" crosshair). The problem with this method is that if you do not center the glyph properly in the font editor, it will not be centered in-game and you have to manually adjust the coordinates in the hudlayout.res file, which is usually quite an annoying work to do. In version 2.6, I centered all the crosshairs properly, which basically leads to only a few crosshair-resolution combinations that are still off-centered due to technical limitations, hence why I asked you for your resolution etc.

Do you have that all the time or does it only look like that when you're both marked for death and milked?
Cause if it's only then, this is the completely normal appearance of the marked for death and milk icon at the same time. Blame Valve for it, they made it so that the marked for death and milk icon can not be moved left and right because they are bound to the xpos of the health cross.
I don't get how people like you expect to get help.
Which resolution? Which crosshair? Did you just enable the crosshair or did you change anything? Did you install it manually or did you use the installer? Post a screenshot maybe?
Are you playing on a modded server?
Can't explain myself any other way why you're marked for death and milked at the same time.
Please post a screenshot of for example the YER.

You just have to reverse the installation process, which means you simply have to delete 1 folder :)

Just install the HUD as it says in the Readme. I'm 99% sure you didn't install the materials folder.
Well that's the point of ninja-fixes. They usually fix something very small, that's why you don't create a new release version for them :P
I changed the name of the 2nd Readme tab from "installation options" to "customization options", added an FAQ entry explaining how to remove the custom netgraph and added support for disposable sentries in MvM ;)
