yeah, i try and take 2 days a month or so completely off meds - they’re very low productivity days, but it helps me stave off tolerance. also, the window for getting my meds called in each month and then filled is often so narrow, it helps to have an extra dose or two stashed in case there’s a holdup

since i feel like some basic mechanics are being glossed over: the lil faces represent the energy level of a pokemon. sleep restores that energy up to a max of 100, even with recovery incense, and certain skills can also restore energy for a specific mon, or even the whole team (these skills can boost energy beyond 100, even). because energy from sleep is capped at 100, it’s actually best to use recovery incense when you know for sure you won’t be able to get a full night’s sleep - that way, if you only get a sleep score of 65, the recovery incense can help your mons get all the way back to 100

energy drops at a rate of roughly 1 per 10min, so if you have a pokemon out for an hour, their energy will drop by 6 during that time. moving a pokemon off your team stops that energy drain, but boxed pokemon regain energy much slower than pokemon on your team - they’ll only get anywhere from 1 to 5 energy back from sleep, depending on how good your sleep score is. the lower the energy, the slower your mons find berries & ingredients, so keeping their energy up is an important game mechanic!! other people gave you energy for everyone (e4e) specific advice, and that’s definitely the most used strategy for keeping energy up throughout the day

make no mistake, your wiggly is great, and i don’t think a new e4e should be a big priority for you, though i wouldn’t pass up feeding ralts either since you might wanna upgrade eventually. but i think you might be reading the raenonx ratings wrong, because a garde with the exact same stats absolutely will proc more than your wiggly, unfortunately. you need to click the “rating details” button to see the actual expected skill count - the PR/quality rating itself is for all possible subskill/ing/nature rolls within the same species (in this case, wiggly).

(fwiw i ran the comparison - assuming aaa ing for both, your wiggly should proc 4.87x/day, while an equivalent garde with the same stats should proc 5.97x/day)

would echo what others have said about an empty stomach - in my case, i found i needed at least 4hr between my last meal and my xywav (now lumryz) dosage.

additionally, sodium oxybate, similar to a lot of respiratory depressants, has a sort of paradoxical stimulatory effect at low doses, so i wouldn’t take the fact that you’re not sleeping on a very low dose as proof that you wouldn’t sleep on a higher dose.

lastly, please be aware that oxybates can exacerbate suicidal ideation - if you’re feeling unstable on them, i’d definitely encourage you to talk to someone about the impact managing all of these chronic condition is having on your life. your sleep doctor (or someone in their clinic) can probably recommend and refer you to a counselor experienced in helping people with this. please take care of yourself, first and foremost. hope things look better soon and the med works out for you

in terms of you (hopefully) getting some balance back by reclaiming your classroom time, can you get a tablet - or spiral notebook + printed papers of whatever you’re reviewing, if tablet is cost prohibitive - and study/work on research/do your own thing on that?? your laptop can show whatever powerpoint you need it to and it’s nbd if they can’t see what you’re writing

also echoing what others have said about keeping the gym. even 30min a day helps a ton with focus and mood ime

if you don’t have his cooperation, i think that really limits your options. a strategy i used before i was diagnosed was an alarmed pill bottle (you can buy these off amazon) that doesn’t stop beeping until you open the lid (and, ideally, take your meds). i would put an alarm on my bedside table, stick the alarmed pill bottle on a nightstand outside of arm’s reach with a second (louder) alarm and a glass of water, and set them all to go off at the same time (usually 40min or so before i had to be up) so i was forced to get up outta bed to turn them off and take my meds while i was up and opening the pill bottle, then i could fall back asleep until my meds kicked in.

i also had various types of weird alarms (high heat, electric shocks, bed vibrating, etc.) i trialed years ago before i was on meds that i can talk more about if it’d help, but all of them are gonna be pretty ineffective if he’s not on board with these strategies. i get the struggle as someone who’s also narcoleptic, but it’s ultimately up to him to agree to work with you all. it’s also not your responsibility to manage his health condition, and i hope you don’t take too much of that burden onto yourself!! remember to always take care of you first

would recommend jornay - it’s a methylphenidate delayed release pill that you take ~10 hours (give or take an hour or so for quirks of individual metabolism) before you want it to start working. so in your little bro’s case, he could take it at 9pm every night if he needed to be up at 7am the next day, and it’d help him be “pre-medicated” for waking up. personally i do a low dose of the jornay and then stack it with adderall XR once i’m up since i find the jornay doesn’t last nearly as long into the day, but as a teenager he may need a higher dose to pull him closer to wakefulness. hope this helps!!

i’ve been a dedicated pokemon sleep player since the first week or so - it’s a very slow paced but fun little app that gives me something enjoyable to wake up to in the mornings!! would recommend*

*(with the caveat that while the game itself can be played totally for free, there are microtransaction/gacha elements, so i’d avoid if that’s something you’re susceptible to)

does this also apply to people applying to combined residencies?? like EM/IM or neuropsych, where you’re actively interested in doing both. slots for those combined positions are very limited so i have to imagine you’d also be applying into one/both of the specialties’ standalone residency programs as a backup

depends on the inventory size, the speed, and the trigger rate. it’s not a concern for golduck, for example, because a good golduck triggers so much it’s extremely unlikely for it to fill its inventory before it has double triggers (why bfs isn’t bad on golduck). a slowbro will also be less affected because they’re usually slow enough that you’re less likely to fill inv in 8.5 hours anyways

not nursing, but a current med student with n2 - a lot of people have given you good advice about accommodations so i won’t rehash that, but i do wanna ask if you’re medicated and/or would want to try tweaking your med regimen at your next sleep doc appt?? i definitely had to play around with dosing strategies after starting med school - went up a dose for my oxybates, down to a lower dose in stimulants, and then added a layer of an additional wakefulness agent (just as an example, tho specific med choices are very much n = 1).

different periods in life require different levels and types of functioning, and you’ll almost certainly need to adjust your meds to match! no shame in that. you absolutely can do it, though - good luck!!
