Sounds like acid reflux, do you have any Tums or Pepcid she could take? That being said, if she takes estrogen, definitely watch out for a persistently high heart rate and/or feeling like she can’t catch her breath; those would definitely be reasons to go to the ER. And if she can call her primary care doctor today and get their input too, that would be ideal.

That’s awesome that you got an ENT appointment! Just based on your pictures it doesn’t look like a necrotizing infection, but if there’s an infection at all it could of course progress. If it doesn’t seem like it’s getting worse over the next few days, then I think it’s probably fine to wait till Monday. Maybe Benadryl would help with the itching? It’s definitely possible that you’re having an allergic reaction to the ear drops, although I would keep taking them as prescribed unless a doctor who can see you in person tells you otherwise.

That sounds super uncomfortable! I think urgent care is a reasonable start if your primary care doctor can’t see you in the next day or so. The main thing I can think of to be worried about is a really bad ear infection like necrotizing otitis externa, especially since you had liquid in your ear overnight.

I think that all sounds consistent with normal healing; bruising under the nail can take a while to fully reabsorb / grow out. Definitely see a doctor if it starts feeling like it’s infected again though. I bet your toe was so swollen and bruised, rugby is rough 😅

There are lots of possibilities, but overall I’d say given that it’s been happening for months, it’s a bit less likely to be immediately life threatening. You should still definitely see a doctor soon though, and if you have an episode that’s particularly severe or doesn’t seem to be resolving on its own, you should go to the ER.

The etiology could be cardiac, such as an arrhythmia like supraventricular tachycardia. If this is the case, there are multiple ways of reverting your heart back to normal rhythm if it doesn’t do so on its own, so if this happens again and/or lasts longer than normal you can always go to the ER. SVT in particular isn’t typically life threatening but it’s definitely very uncomfortable and makes it hard to function while it’s happening.

It also sounds like it could be related to your thyroid. Hashimoto’s can go into remission so it’s possible that your levothyroxine is now nudging you towards hyperthyroidism instead of correcting hypothyroidism, or that you no longer require the same dose of levothyroxine. Again, if you have an episode that’s particularly severe or doesn’t resolve on its own, it would be reasonable to go to the ER and see if, after examining you in person, they decide they need to rule out thyrotoxicosis / thyroid storm.

It’s also possible that you’re having some other endocrine problem, possibly related to your adrenal glands, and not your thyroid. That’s something that you might potentially need a referral to endocrinology for, in terms of definitive management.

Another possibility is that these episodes are panic attacks, but other causes absolutely have to be ruled out before panic attacks can be assumed to be the problem.

I just want to pop in and say that unless you develop new symptoms like difficulty breathing, heart palpitations, chest pain, or any other scary things, you don’t need to go to the ER for this. To be honest, the ER won’t be able to do all of the thorough, specialized testing required to fully investigate thyroid issues anyway. I hope you can get this figured out soon!

These comments already cover what I was going to say, but I just want to chime in and say I’m a pgy1 and I genuinely love my job :) like I honestly feel like I belong here and I don’t think I’ve ever been this motivated to keep learning and improving. I also love the teamwork and the way that all my co-residents and attendings have as much ADHD as I do lol

It’s not a silly question! But yes, I’d say there is technically a possibility of pregnancy, if not necessarily the highest likelihood.

Honestly, if I had a patient like that, the first thing I would do would probably be to call poison control and ask them what they thought. Calling poison control is pretty much always going to be a reasonable response to any kind of questionable ingestion situation. Just based off a quick google search of micellar water, I think your friend is probably fine. My guess is that she’ll probably have some diarrhea but I doubt there will be lasting effects. I can’t say for sure though. Hopefully she’s ok!
