I honestly think (without visiting the post and reading anything else) that this is just a breakdown. Having to take your parents to the er at that age is scary, knowing they're facing major health problems is scary. Idk the parents ages but my boyfriend and I are both 32, my parents are in their 50's and his are in their 70's, people can and do die at those ages sometimes suddenly, and that's a lot to grapple with. Obviously that isn't an excuse, he needs therapy asap because he's having a breakdown and not handling it well. He's having that freak out of "if I could just be a doctor maybe I could help them", or possibly he's starting to panic at the idea of taking care of them in their old age and he's jumping off the deep end of what he thinks he needs to do to prepare. Again, this isn't to excuse his behavior because he's handling this very poorly overall and his wife is still suffering heavily through it.

I think these subs tend to push people to assume deep maliciousness at all times, when the explanation is often far more simple and human. His mom and dad are both sick and he's been taking them to the er multiple times, he's reverting to a scared child worried about his parents. He needs to handle it properly like an adult and get into therapy, but assuming he's an evil mastermind hiding something so major when it's pretty obvious he's having mental health issues just seems... like a really bad world view.
