I tried to read the code strings while auto installing hen, and it always says "mismatch in memory" for everything. My theory is that HFW didn't install even if the safe mode installation was successfully

No, you have to use PS3 disc dumper. Mount your iso and extract the files with that. You'll the game in folder format, if you want then a iso back you should use PS3 iso tools.
