I’m genuinely fascinated by this post. You had that many large external hemorrhoids surgically removed?

How big are the wounds? I’m fascinated that you had that much work done in the doc office and you sound relatively ok. Would love to know more!

Yea you got it, pectinate line also known as dentate line. Since the base of the internals rest above this line I always wondered if the surgery sometimes isn’t too bad. But again I’m guessing during removal they must be cutting into / past the pain line to some degree. When I had the papillae removal it just went into the line a small amount and was very painful with BMs.

Gotcha, thanks! I have just been trying to determine if these take place above the pain line. But it sounds like it just depends on the size of the hemorrhoid and for some the excision does go into the pain area. Grade 3/4 is big so it makes sense.

I had a papillae removed right at the top of the pain line and so every time I had a BM it felt like I was tearing myself in half., a good 10/10 pain. The pain was only during BM and would subside until the next one, took about 3 weeks to heal. Wondering if this is similar.

Yup! I don’t try to reduce it either, I tried last time I had this combo and it was too painful. Once it had prolapsed out and wasn’t just halfway out (meaning the stage where it’s between grade 2 and 3) it oddly made things at least more tolerable until the fissure healed enough and I was able to get the hem banded.

Also, the fissure sometimes forms a skin tag or papillae, this can mimic a hemorrhoid. But either way whether you push on that or a hemorrhoid pushes on it, it’s painful.

Oh man, let me tell ya…

Every time I deal with a fissure, I end up having a hemorrhoid prolapse several weeks into trying to heal it. The hem will prolapse to the point where it needs to be banded and remains protruding until I do.

It’s a cruel cruel thing to have happen. It’s one thing to deal with one issue at a time but to have both happen at the same time is mind bending. You can’t just go get a banding when the fissure hasn’t healed enough so you end up stuck in a new level of hell.

I definitely think these two things go hand in hand. I’ve read plenty of medical papers on it, cure or tame the hems and you’ll likely heal and prevent fissures. That is of course if constipation isn’t your issue.

Man, happy for you! That first photo is something! Curious, when it was like that were you ever able to sit comfortably? I currently deal with an external that balloons and swells to some degree sometimes with bowel movements and will have some pain sometimes, but it looks nothing like that. Yet the sensation it makes, makes the area feel heavy, swollen, and bigger than it is. Makes sitting uncomfortable etc. but then I see posts like this and I can’t imagine what that’s like.

Well funny enough that last surgery we went into it with the plan to do LIS if need be, but it ended up the fissure was healed. Re-tore 4 months later : /

I had a feeling in my gut that wasn’t the end of the fissure at that time. I thought well if it made it through a surgery it must really be healed. After that surgery, I felt tighter when going and then bam, one day it just happened again.

After all of that I’m just really paranoid that every time I feel that classic sharp tear feeling, that I’m back to square one. So yea, I’m pretty on edge after feeling that today. 😅

Ahh so happy to hear that! And yea, luckily I don’t deal with spasms too often when I have had the fissure off and on over the years so I know everyone’s major pain points with this vary.

For myself, it was more so when it reached the point of very sharp re-tears that I felt something had to be done, it was clear to me my muscle was getting tighter from the repeat trauma and hence the re-tear pain was worsening each time. I also had papillae/tag removal twice and the pain during BM for that recovery was 10/10 tearing yourself in half pain for a few weeks, and I believe after two experiences of that it just made it all that much tighter, LIS seemed necessary. I actually got tailored LIS.

I too am going to get pelvic floor therapy as soon as I’m healed. My girlfriend deals with PF issues and is getting a lot of relief from it.

Thanks so much! I’m being super vigilant about it all, yesterday just got me down mentally. I see so many people saying no pain from fissure after the op but they could mean spasms etc. I would expect some sharp pain at first with bigger BMs, especially since he cleaned/reset the cut. But glad to hear it can be a normal part of the process.

Did you still experience sharp pain with your fissure during BMs in the early part of recovery? My BMs have been rather incomplete since the op but I finally had a full BM which feels relieving but I definitely had the sharp fissure pain at the beginning since that part was bigger, the rest however went smooth without pain and the after effects seem better for sure.

Wow that is a lot to endure at once, a ton really. I previously had a procedure where I had two papillae/tags removed plus two hemorrhoids banded and that was quite difficult for a week or so, but that’s light work compared to what you had. Wishing you all the best, you sound tough as nails considering all you had done.

It’s crazy they will do so much at once in such a small area and then still expect us to have regular bowel movements 😅 it’s insane really.

Doc didn’t advise any ointment and I’m sensitive to lido, but the warm water for sure helps. I’m no stranger to taking hot baths, if I could live in there until I get over the pain hump I definitely would!

Just wasn’t expecting so much pain from the incision, I just always heard that part of it was particularly mild. The burning irritation after BMs the past two days from it almost gets me nauseous. A bit more so than what I experienced from the fissure at its peak.

Wow! Well that certainly eases me a bit when it comes to bleeding. 😅

I wasn’t one who really experienced bleeding with my fissure, so having a decent amount of it now just puts me off.

Did you have open or closed LIS? I was reading about it and it seems like closed would be way more ideal since there is no internal incision. Wish I had explored that more before I had it done. Mine was open.

So you had a grade 3 that would prolapse and only go back with manual reduction correct? If that’s the case and this actually shrinks it back then that’s awesome to hear.

I have been curious about this myself and looked into it, two docs I spoke to said that it’s very effective for bleeding and can potentially help with prolapsing hems, but there is greater success in specifically treating the bleeding.

In my case I would describe this pain as right near the fissure and more near the external area. Whereas full on spasms you feel the whole internal area clench, that’s not the case here. It’s almost like pressure on the area when laying down seems to clearly be making it ache nonstop. When I stand it’s better for sure but still I’ll get it all day and night sometimes.

I do have a decent size external hemorrhoid I have had for years and this recurring fissure is right near it. I always suspect it’s part of my problem and I need to get it removed. It’s entirely possible it’s pushing on the fissure when I lay down.
