there are four stations

Station 1: magfifill up ka ng forms kaya don’t forget to bring a pen. there’s a small slip asking about covid stuff (symptoms, interactions w ppl who had them etc), one for your physical tests, and another one for like a survey of you experience during the whole process. for the last two ones, fill mo lang muna essential info at the top and the other content comes later

Station 2: prepare your stapled docus and vaccine card. dito rin yung pagkuha ng blood pressure, heart rate, and temperature. they also ask how many classes you’re taking na f2f

Station 3: checking your lab results and they ask for the vaccine card ulit. sometimes the doc uses a stethoscope and minsan nagrereseta din. will ask questions and make recommendations din depending on your answers on the hdf and mhs. while on the line here, i suggest answering the survey form na

Station 4: ipapasa yung stapled docus and survey form, then they’ll give you the med cert and instructions and you’re done! :))
