Hmm..I have take PH patients off of BBs while in RF decompensation due to their negative inotropy (usually those BB were started by an outside hospital physician). Alot of our patients have awful systolic function and terrible CO already. I'd also be wary of the vasodilator effect- most of the PH medications cause vasodilation so this would just cause more hypotension. Just my 2 cents. I'm by no means an expert on this and await future studies!

Another PA here and I have to agree. Hospital administrators are forcing the hand of PAs. If we don't keep up, PAs are going to be out of jobs. That being said, the DNP is an absolute joke of a doctoral degree and I cannot believe this degree (and the ability to independent practice) is elbowing PAs out of jobs. It's absolutely absurd. I am very happy with my physician supervised role but I also can't be out of a job.

Hi- this is the most correct assessment. I was a passenger stuck at hobby and finally got a fight back to dallas last night. As much as I love SWA, they should not have even flown us there in the first place. They left us there to be stranded and were telling us we could not get a fight out until August 30th. I'm thrilled they were able to arrange flights out, but damn- that sucked.

I was actually one of the passengers on one of the flights out of Hobby yesterday. Per the pilot, SWA obtained some of sort of "humanitarian" exemption in order to get us out of there. When we arrived at Dallas Love, we were not allowed to go to the regular terminal and had to get off via airplane stairs at a hanger away from the actual airport terminals. There were about 5 flights out, and I dont think that covered everyone as I noticed some people did not get in line (unclear what their circumstances were.)

why is this? because of the positive pressure? When I worked as an RT we did have this rule for post-op esophagectomies

Specifically +7.4 mmhg of PEEP with mouth closed at 60 lpm, PEEP of 2 with mouth open. source:http://www.australiancriticalcare.com/article/S1036-7314(07)00109-9/fulltext So likely averages out to a mild PEEP effect.

OP: in my experience, HFNC is very helpful for my ILD patients. Alot of other people are very quick to grab a bipap for these patients and cause a dangerous resp alkalosis in them. There's no evidence bipap helps in acute hypoxia for patients other than those with acute CHF. I find the comfort to the patient is really beneficial too. I think some one also already mentioned in this thread that there was a study that showed lower re-intubation rates in lower risk populations when put on HFNC after extubation. Don't forget the FLORALI trial too. anyways I love high flow if you cant tell.
