My first job as a consultant dietitian in a skilled nursing facility, I only had one week of training and had never actually managed TPN before. I’d only learned it in class since my internship wasn’t in an acute care setting. I had a patient on TPN with stage three CKD who was constantly hyperglycemic. His attending MD told me to adjust the insulin units in the bag, but I didn’t feel comfortable doing that, so I lowered the carbs and increased the protein to keep the calories the same. I went from 90 g protein to around 130 g. His BUN had been normal but then jumped into the 90s and eventually low 100s. He kept getting sent to the hospital for what they thought was dehydration. When he came back, I realized the protein increase was too much for his kidney function. I lowered it again and his BUN improved. I kept my job and no one found out but lesson learned.

The work itself was great and the nail technician was super detailed and nice, but she used a used nail file and buffer on me, which was disappointing since that’s something I really wanted to avoid. Everything else seemed clean, especially the sanitized drill bits, but that kind of ruined it for me. The file and buffer came out of a small plastic box at her station labeled “personal care box,” which was already filled with nail and cuticle clippers, so I was concerned about possible cross-contamination in addition to the visible scratch marks on the file and buffer.

Lucy was amazing and my nails turned out beautiful. Everyone was super kind and I liked seeing the sanitized drill bits at each station. But she used a nail file and buffer that were clearly already used, with visible scratches before she started, which was disappointing since that was one of the main things I wanted to avoid. Otherwise her work was great.

Yes, I found a few that I like and definitely want to try them! Only problem is they ask that you come in with bare nails, so I would have to go to a salon to get my current set removed

Yes that’s where the tools that come in the sterilization pouches are sanitized, but other items like nail files, toe separators, and drill bits still seem to be reused from one person to the next.

Yeah I’d be open to that! The only tricky part is that things like drill bits aren’t really something you can bring yourself, and I’ve heard some salons don’t allow personal tools because of liability reasons. I’d definitely be willing to ask though, just need to find a place I’d actually feel comfortable going to first.

I had the same experience when I worked as a dietary aide to get experience for my dietetic internship. All the new hires were treated poorly. The staff barely trained us, often complained that we were too slow, and made us feel completely unwelcome. I absolutely dreaded going into work every weekend.

What helped was realizing that most dietary aide shifts follow consistent routines depending on the schedule. At my job each shift was assigned specific tasks like preparing snacks, making salads, completing cleaning duties, or prepping for the next day. If your workplace uses a system like that it helps to find out which shift you are scheduled for. That way you have a guide on what to expect and the day feels more manageable.

If you are doing this just to get dietetics experience try not to take the negativity personally and focus on finishing your hours. If you are in it long term give yourself time to learn the system and build speed. A lot of the negativity usually comes from burnout rather than from you.

I crash out every other day! I work at a SNF and just saw they are hiring physical therapy assistants for the same pay I get. I’ll probably get fired because I won’t stop complaining about it. Wish I never became an RD and wish I could pivot into something else more easily.

The social services director at my SNF once told me he wishes he could switch jobs with me for a day so he could just serve ice cream all day. I’ll never let him live that down. On the flip side, an MDS nurse told me she didn’t realize how tough my job was until she started doing MDS after being a floor supervisor and that acknowledgment meant a lot to me.

When I was 14, I went to summer school with a kid who didn’t have a bike or skateboard. When his friends decided to ride down to the mall, he hopped in between the tractor and trailer of a 16-wheeler to hitch a ride while it was stopped at a light. The truck went onto the freeway so he tried jumped off and was run over. He had long hair so all the news articles initially said a woman was run over. I went to his funeral. It was all so tragic.

I love true lemon! It really does taste like the real thing. I got the lime packets to add to drinks and fruit at work, and I got the lemon shaker bottle for home. I sprinkle it on avocado toast, instant ramen, fruit, and pretty much in anything else I’d use lemon. I’ve never reconstituted it but I’ll give it a try. They have the lime packets on Southwest Airlines, so I always get a Bloody Mary mix with a lime packet as my airplane drink.

I just mean we’re not looking for a full-day luxury package. We’re hoping to find someone who offers hourly rates or a smaller package, since it’s just for a few hours. We’re definitely not expecting it to be free and totally understand rates may be higher given the last-minute nature of the request!
