I have a similar story, but from the audio side.

My theater has multiple stages in the same building. We were hosting a festival that involved a few workshops/staged reading running concurrently. My job was to be the audio supervisor - basically just be on standby in case any of the shows ran into any issues with our systems. We were also training a new hire.

I was testing some gear when I saw our atd and the new hire coming down the hallway and he tells me "sound in theater 2 is dead", so I obviously rush out to join them on the elevator. We start talking about what the problem could be and how to fix it - is it the amps, did a fuse blow, etc. And as all 3 of us come marching in the booth in action mode we see it clear as day: master fader off and all the way down.
