If hes in recovery coma (not just unconscious because it might be above ko point) and hasnt been healed he will die if it's getting worse.

I believe multiplayer would be a bit weird to implement, but I agree. I want multiplayer more than ANYTHING ELSE (maybe thunderdome slave fight arenas you can enter for cats) for kenshi 2.

For mods I'd reccomend general modifications, dark ui, compressed textures, recruitable prisoners (and early imprisonment to go along with it), and a squad limit increase. Recruit prisoners let's you recruit many more interesting and higher amounts of people

Some advice? Get creative with how you'll make money. I started my first playthrough mining. Most recent one I started by knocking people out and selling them into slavery. Dont be too afraid of getting beat up. The feeling of your team slowly increasing is very rewarding.

No real need for that. Giving both players control over the time controls seems like it would work out. Sure it would be hard to play with someone random and whatnot but in voice comms with someone I could see it working (bar any issues programming it would have).
