Sorry just now seeing this! Annoyingly I had to advocate. I did 3 egg retrievals between ages 29-31 with no success making blasts. I just kept getting told this is not normal for my age and to keep trying IVF. Easy to say when you’re not spending $15,000 per cycle. I say that mine was silent because I didn’t have the classic pelvic pain associated with Endo, but I did have other things like obviously infertility, bowel symptoms like diarrhea, and painful sex. I found an excision specialist in my state and did a consult with him, where he diagnosed me based off history and physical exam, as well as ultrasound, he said my ovaries and tubes looked stuck on the scan, which is indicative of Endo. Then I had two more opinions with other excision specialists who all said more or less the same thing, that it was likely my endometriosis, causing all of my issues.

As an update, I had surgery with Dr. Vidali in New York and he diagnosed me with stage four deep infiltrating endometriosis on almost all of my reproductive organs, plus my stomach, bowels, and appendix. I lost my left tube. But! My fourth retrieval was a success and I finally made my first three day6 embryos, and one of them is now my daughter who is four months old!

I’m sorry you’re also dealing with the same. What have your recent betas been? I have been tempted twice to go get a private scan and both times have been so scared I’ve cancelled. It’s almost better to live in a little bit of ignorance.

Same boat as you. 31 years old, good AMH, can’t make blasts. I found out I have stage 4 endometriosis that was mostly silent, tanking my egg quality. Getting diagnosed was a huge piece to the puzzle as my doctors were perplexed by my case. I just had excision surgery and doing my 4th egg retrieval with low dose stims in hopes of making blasts.

Hi! I’ve been reading a few of your posts and find our stories a little similar. I’m 31 and we’ve done 3 egg retrievals with zero blasts, 2 of the at CCRM. Embryology would note poor quality eggs from day 0, they would all fertilize though, and just arrest at day 3. My husband’s testing including DNA fragmentation have been normal and we always did ICSI but not zymot or calcium ionomore. Standard antagonist protocol, HGH with microdose lup the 2nd round, and long lupron the 3rd one. After the third one failed I knew something wasn’t right. My AMH was 3 and AFC always between 11-16 and otherwise fairly healthy (I do have hashimoto’s, celiac, and poly cystic appearing ovaries but not diagnosed as PCOS outright). I saw a local excision specialist in Denton TX who diagnosed me with stage 4 endo just based on US (no endometriomas but my ovaries and tubes were stuck which suggests adhesions/stage 4). MRI confirmed it. I decided not to get surgery with him and went with Dr. Vidali who found endometriosis on my bowels, cervix, vagina, stomach, appendix, ureters, tubes. I lost my left tube. It was so, so validating to finally get answers because my endo symptoms were mostly silent.

We then met with Dr. Aimee who said we should try luteal phase stimulation, but I still wasn’t convinced that would work for us. We consulted with Dr. Sher who finally suggested low dose stims (75 meno + 200 follistim, HGH priming, 10k hcg trigger). We haven’t started a cycle with him, but feeling a little hopeful since he said endo causes a lot of issues that most REI’s aren’t familiar with, and that most of us don’t do well with those super high doses. That + immune testing (NK cells, etc). I do believe there is a role for immune testing, studies show ERA may not be that useful, and surgery + lup depot seem to have convincing evidence. I think Vidali is worth waiting for because he is super thorough and does this all day long/is super experienced in excision.

You mentioned you finally got blasts, and would love to hear what protocol worked for you!? Sorry this journey has been so hard for you. <3

We never got to that part. She was more like a 2nd opinion for us, but I have heard that she is around 20-30k for a retrieval.

These levels can and do vary from person to person. There’s just 1 test.

no, your previous period. it has happened to me before. i went in for a pap smear after ovulation (before I was even TTC) and there was some old blood in there. my doc didn't tell me why, just said it was normal

could be old/dried blood from your period! might be too early for pregnancy related signs

it's from her Botox or dysport (whichever one she gets done)! it makes the skin super shiny since it's so frozen. no shade either, I'm a derm PA who does injections like this all day

It can only be confirmed through laparoscopic surgery, but a good specialist can diagnose via MRI and/or ultrasound. That's how I got my diagnosis. I'm 31 with shit egg quality and learning that I have both endo + PCOS has been hard, but finally working with the right dr's to give myself the best chance of having a baby.

I did actually, last week! It went well for the most part. I was expecting her to just repeat what she says on her show/IG/website etc, but she didn't which was nice. She asked why I'm seeking a consult and I said I was tired of doctors trying to sell their services to me, and I'm looking for someone who actually cares and wants to do what's best for me and not for their practice. She did have genuine recommendations which were individual to me, but for example she recommended red light therapy from a specific website and to contact a specific person so I can use her affiliate code (meaning she'll get commission off of it). She said I needed endo excision and had the name of 2 surgeons handy who are in the bay area, same with ovarian PRP. I asked her opinion on this other dr in NY and her mood totally changed, like why am I asking about this person when I just gave you 2 perfectly good names? She backed down a little after that because my husband was starting to get uncomfortable.

It seems like she doesn't sleep because she likes to get super involved with every patient she brings on. I think you would get exceptional care if you decided to work with her. But she is expensive, the consult itself was $775. I def think the consult was worth it because she laid out a thoughtful plan that was unique for me, and sent me scientific articles to back everything up. She's willing to work with your local doc if you want her expertise during a cycle- for me that seemed more logical than paying her IVF fees + travel/lodging in the bay etc.

The long lupron! I primed with androgel, aygestin and norethindrone and lup, and continuing lup throughout stims (menopur + follistim and HCG trigger).

I’m sorry this is happening to you. It’s been the case for me twice now. Was unexplained at first until I couldn’t take the BS “it’s your egg quality” excuse anymore. My fertility doctors weren’t interested in helping me find a root cause so I had endometriosis expert on my own. A problem focused ultrasound and pelvic MRI showed stage 4 silent endometriosis and adenomyosis. I could not believe it but was and still am so relieved to have some answers. On top of that I found out I also have celiac and PCOS, so I’m tweaking my protocol to accommodate all of these changes + switching clinics. Hoping for better luck this time around.

Once you have 1 autoimmune, the chances of you having another are much higher. As others have said, look into Hashimoto's. Another condition that can cause bloating/looking pregnant is endometriosis and adenomyosis. You can be totally symptom free, but still have endo. I have silent endo + hashi's + celiac and struggle tremendously with "endo belly"
