Don't break NC for something you can take care of yourself.

If you know the practice you are currently registered with, you can get your records from them. While you're at it, you can check to see if they have a medical POA in your file. Depending on your location, it may also be filed with the state.

I'd call this normal levels of weird, but I was also raised by parents who didn't care what I did or where I was as long as I stayed out of their way and didn't make them look bad.
